Abstract:
This medical case study outlines a successful inter-state critical care patient transfer involving a 63-
year-old patient with a complex medical history. The patient, diagnosed with cerebrovascular accident
(CVA), Parkinsonism, and diabetes, experienced respiratory deterioration necessitating advanced
ventilatory support. Facing challenges such as geographic distance, transportation limitations, and
patient instability, a comprehensive solution was implemented by the healthcare logistics company,
HANUMAN. This case study highlights the efficient coordination of medical equipment, paramedics,
and virtual medical supervision, resulting in a safe and seamless transfer from Bhuvneshwar to
NIMHANS Bangalore. The innovative approach offered a hassle-free, cost-effective, and timely solution
for inter-state critical care patient transportation.

Introduction:
Inter-state transportation of critically ill patients presents significant challenges due to geographic
distances, patient instability, and logistical constraints. In this case study, we present a unique solution
devised by HANUMAN, a healthcare logistics company, to efficiently transfer a complex patient from
Bhuvneshwar to NIMHANS Bangalore.

Patient Presentation:
A 63-year-old patient with a history of CVA, Parkinsonism, and diabetes was admitted to an ICU in
Bhuvneshwar with severe tachypnoea. Mechanical ventilatory support was initiated, and the patient
was subsequently placed on BiPap for over 10 days. Hemodynamic instability, respiratory risks, and a
developing bedsore necessitated a transfer to a higher-level neurological care hospital in Bangalore.

Challenges:

❖ Patient's critical condition requiring high-flow oxygen, advanced monitoring, and medical

Solution:
HANUMAN, a healthcare logistics company specializing in critical care patient transfer, was engaged to
provide a comprehensive solution. A multidisciplinary team including a Senior Critical Care paramedic,
virtual doctor, and specialized medical equipment was assembled to ensure a safe and efficient
transfer.

Methods:
❖ Assessment and Planning: A visit was arranged to assess the patient's condition, medical
needs, and transport risks. The paramedic collaborated with the treating doctor to plan
enroute care, medication, and emergency protocols.
❖ Equipment and Medical Team: The patient was accompanied by a Senior Critical Care
paramedic, a 5-parameter patient monitor, a ventilator, three infusion pumps, a backrest, a
suction machine, an air mattress, four jumbo oxygen cylinders, and essential emergency drugs.
❖ Virtual Medical Supervision: Enroute care was closely monitored by a virtual doctor using
IoT devices, including the patient monitor and stethoscope. Real-time communication allowed
prompt medical intervention if needed.
❖ Logistical Support: HANUMAN managed all logistical aspects, from ticketing to medical
equipment arrangement, ensuring a hassle-free experience for the patient's relatives.

Results:
The patient's transfer from Bhuvneshwar to NIMHANS Bangalore was successfully executed without
any significant complications. The comprehensive approach, incorporating advanced medical
equipment, expert paramedics, and virtual medical supervision, ensured the patient's stability
throughout the journey.

Discussion:
