Notification of Blanket Waiver

The Indiana Department of Workforce Development (DWD) is providing notice that some or all of your
overpayment for the Pandemic Unemployment Assistance (PUA) Program and/or the Federal Pandemic
Unemployment Compensation (FPUC) Program has been waived.

The PUA and FPUC programs were created to provide additional benefits to unemployed workers
impacted by the COVID-19 pandemic. Please see our COVID FAQs for more information about these
programs: Unemployment.in.gov, choose Frequently Asked Questions, click to download COVID-19 UI
Frequently Asked Questions for Claimants. You are receiving a waiver because your PUA and/or FPUC
overpayment meets one or both below requirements for a blanket waiver, pursuant to Unemployment
Insurance Program Letter (UIPL) No. 20-21:

1. You answered, “None of these options apply to me” when asked to choose at least one
qualifying COVID-19 reason for being unemployed on the Federal Pandemic Unemployment
Assistance Application and/or weekly voucher(s); AND/OR
2. You answered “No” when asked “Would you be able to work and available to work at least part
of any day?” on the Federal Pandemic Unemployment Assistance Application.

Please note this waiver only applies to the PUA and/or FPUC programs for the stated reasons above. It
does not apply to any other overpayment(s) you might have. If you have applied for a waiver of an
overpayment of any other program, DWD will respond to that waiver request after it has been
reviewed.

Based on the above, you are no longer required to repay the PUA overpayment for benefit weeks
ending 12/12/2020 through 12/19/2020, and 02/13/2021 through 07/24/2021, in the amount of
$3740.

Based on the above, you are no longer required to repay the FPUC overpayment for benefit week
endings 02/13/2021 through 07/24/2021 in the amount of $7,200.

There is nothing more for you to do to get this waiver. If you do not want a waiver of the overpayment
or if you have any questions, please contact the DWD Collections Department at (800) 262-6949.

Sincerely,

Indiana Department of Workforce Development
