Dear Dr .Brown,

Re: Mr David Taylor , coma Aged: 38 years

I am writing to refer Mr. David Taylor,a 38 years old male having 3 children, for the removal of damaged
cartilage of his left knee and further management if required.

On his Mr Taylor’s first visit on 12/8/10, coma he presented with the complaints of pain and swelling in
the left knee joint .This occurred because of twisting of the joint 6 months ago while playing tennis.he
was unable to strengthen strengthen his involved leg. On examination, the joint was swollen and had a
tender spot on the medical side for which an X- ray was ordered also done. He was prescribed with
voltarin 50mg twice a day along with the an advice of smoking cessation.

On the next visit, his Mr Taylor’s condition was not improved resolved due to which he was unable to go
for work. There was marked swelling and his range of power was also impaired so ;therefore, an MRI was
ordered.

On today's visit, there is still no improvement and the MRI shows damaged medial cartilage with no
further complications.

In view of the above, i recommend that his demaged cartilage should get remove under your care as it is
not responding to medical treatment.

Given the above , I would appreciate if you could take Mr Taylor under your care.

In case of any queries, please feel free to contact me.

Thank you
Yours sincerely,

Doctor.
