AFFIDAVIT OF ONE OF THE SAME PERSON

I, LORETO S. LACORTE, of legal age, Filipino, with residential address

at Brgy. Sico, Lipa City, Batangas, after having sworn in accordance with
law, do hereby depose and state that:
1. I have been known by the name RITO LACORTE and have been
accustomed to using the same in my personal and official records,
transactions, and communications, as shown by the following:

a. Senior Citizen (Lipa City) Card: 131-31560, issued by the Local

Government Unit of Lipa City, a copy of which is attached hereto as
“Annex A”.

2. My name was recorded as LORETO S. LACORTE on the Transfer

Certificate of Title No. 112676, a copy of which is attached hereto as
“Annex B”;

3. Both the name LORETO S. LACORTE and RITO LACORTE refer to

one and the same person, the herein Affiant;

4. I am executing this affidavit to attest the truth of the foregoing and for
whatever legal purpose it may serve.

IN WITNESS WHEREOF, the parties have hereunto set their hands this

Notarial Practice and acknowledge that the same are their own free and
voluntary act.

This instrument consisting of two (2) pages including this page upon which
acknowledgement is written and had been signed by the parties and their
instrumental witnesses on each and every page thereof and refers to the
Affidavit of One of the Same Person.
