1. I am an applicant for Fire Office 1 at BFP Region 9 and possesses the

minimum qualifications standard for appointment;
2. I have not been dishonourably discharged of dismissal for cause from
previous employment;
3. I have never been convicted by final judgment of an offense or crime
involving moral turpitude;
4. I have read and checked all entries appearing in my Personal Data Sheet
(PDS) and that I certify that all entries are true and correct;
5. I attest to the truth, accuracy and genuineness of all the information,
documents and records contained and attached to this application and that I
hold myself administratively/criminally liable for DISHONESTY and PERJURY
for any misrepresentation or omission, fraudulent declarations, falsifications or
tampered documents that I have submitted;
6. I am submitting this application fully aware that I have to undergo and pass all
the stages of the recruitment process (i.e Interview, Physical Agility Test,
Neuropsychiatric Examination, Medical/Physical Examination) and shall
uphold the integrity thereof by not soliciting political patronage, influence,
peddling or pledging for gifts/ and or monetary considerations in exchange for
appointment;
7. I further certify that I am in good physical condition to undergo any strenuous
activity and shall disclose all matters pertaining to my health status during my
Medical/Physical Examination.
8. I shall be a bearer for truth and justice and shall report any incident or
applicant/individual/BFP personnel based on personal knowledge who shall
engage on the prohibited act/s mentioned in para 6 hereof.
