This agreement serves as a binding contract between Zhairyne J. Montemor,

hereafter referred to as “Student” and Mados Beauty Salon, hereafter referred as
“Association”. By signing this contract, the student accepts the Mados Beauty Salon
Scholarship and agrees to adhere to the following requirements until the end of the
_______. The Student further understands that a violation of the following
requirements will result in a termination of the scholarship.

Student Terms

The Student is being awarded the aforementioned scholarship for the express
purpose of supporting her Junior High School studies. To keep the scholarship, the
Student will do the following:

 Maintain a grade of _______.

 Complete Attendance
 Complete all assignments and take or make up all tests.
 Behave according to the School Code of Conduct. Any infractions may result in
loss of the scholarship.
 Expense the money for schools’ projects and other related school matters.

Association Terms

The Association will award the Student with the total 10, 000 pesos for the
initial school allowance for the school year 2023-2024. Furthermore, the amount
of 2,000 pesos will be given per month for school allowance to her bank account.
