 Accuracy
As a Document Coordinator you are
responsible for receiving, processing and
submitting home care clinical therapy
documents in a timely manner, as well as
reviewing all documents for accuracy and
completeness.

DUTIES AND RESPONSIBILITIES


Every effort has been made to keep your job
description as complete as possible. However, in

 Receives, processes and submits home


care clinical therapy documents in a
timely manner.
 Reviews all documents for accuracy and
completeness.
 Follows up via phone/email with
therapy providers regarding missing
documents and corrections.
 Provide education to providers as
needed regarding documentation.
 Submits needed documents to Home
Care agencies when requested.
 Processes invoices to Home Care
agencies.
 Provides coverage for other members
of the home care team as needed.
 Provides other related duties as
assigned and requested.
 DC is really only receiving and
processing documents- see manuals.
The other items we have split to
different position titles. Up to you how
you want to do JD- as often we train DC
just for doc review and attachment and
then cross train for other tasks
 Diligently pursue deep understanding of
