NOMINATION FORM – FOR LUMP SUM DEATH BENEFITS

While we appreciate, death is a subject we don’t like to talk about, as an employee of Marks & Spencer you will
have the benefit of life cover which is a valuable financial protection for your family should you die while you’re still
employed by M&S. (up to age 70 only).

Please complete the details below to tell the Trustees of the people or organisations that you would like to be
considered to receive any lump sum payments that may become due, in the event of your death.

The amount of this benefit varies depending on whether you are contributing to the Your M&S Pension Saving plan
or not. You can find further information about the Pension Scheme at

Please note, the Trustees would decide not to make a payment direct to someone you’ve nominated who is
under 18 years of age but instead make a payment to their legal representative or into a trust on their behalf.

In the event of any lump sum becoming payable under the Scheme, I would like the Trustees to consider

• This nomination will be treated in the strictest confidence; the information will only be passed to the
Trustees in the event of your death.

• Although the Trustees would normally expect to take your wishes into account, it must be understood
that they are under no obligation to do so and the final decision must be theirs.

- Inform the nominee(s) of your decision.
- Inform the nominee(s) of the discretionary nature of this/these benefits
- Update your wishes on MyHR of any changes to your circumstances which may affect this
nomination.
