OBSERVATIONS
Person B presented for the evaluation wearing their school uniform. His grooming appeared
adequate. His activity level during his evaluation was considerably normal, his speech was clear
yetsome of his words are inaudible and he has a low voice. Some unusual mannerisms were
noted such as tapping of his fingers and biting his fingers. His eye contact was within
considerably normal limits for the reason that he has a covertly and stealthy look during the
interview. No overt indicators of aggression or impulsivity were noted during the evaluation.
Person Bs mood and affect were observed to be normal and somewhat restricted. He seemed
relax yet somewhat tense, he was leaning forward when listening to instructions and when he
was asked questions. Although he kept looking at his peers, he showed willingness to cooperate
throughout the evaluation.
FAMILY HISTORY
Person B is the eldest child in his family and has a younger brother. He is a 13 year old
teenager who is single and currently residing with his family. His mother is a housewife while his
father is a personal driver of a well-endowed family. It can be noted that Person B has a positive
relationship with his parents, especially so with his father who he sees as an inspiration.
PHYSICAL OBSERVATION
Person B is a typical 13 year old student who stands younger for his age and build. He
appears to be in slouched posture and has normal. There are no physical disabilities and
abnormalities observed. At the time of examination and referral, Person B can be consider in a
normal functioning state. It was observed during the test administration that he is considerably
composed and well-mannered in the time of observation and examination.
MEDICAL HISTORY
Person B is currently taking drugs for his teeth problem. Although not clearly elaborated by
the client, he seems to be taking another medication pertaining to his head.

very hardworking and careful person. He tends to meet deadlines or does things with dedication
and honesty.

PSYCHOMETRIC TESTS
Person B got a high score on Conscientiousness , an average score on Neuroticism,
Below Average Scores on both Openness to experience and Agreeableness and
Low score on Extraversion.
DAPT
Person B’s DAPT showed themes of masculine assertion, active sexual lifestyle,
mainly masturbation, dependence on mother, feelings of inferiority as well as state of
orderliness and comprehensiveness. Moreover , he also shows signs of withdrawal from
reality.
BENDER VISUAL GESTALT MOTOR TEST
Person B has a probability of having problems regarding his visual perception due
to his u usually large drawings, but has no issues pertaining his motor skills, albeit his
habit of tapping his fingers during the examination may men something.
Conclusion
Person B came to all tests administration with cooperation and willingness. Though the tests
he took points at the main theme of being organized and orderly, for it is supported by his result in
his DAPT and high score on conscientiousness and low score on agreeableness, moreover it is
observed on his behavior while taking bender gestalt visual motor test that he would mostly
conduct erasures and flipping of cards. These symptoms are early indicators that this patient
may be susceptible to OCPD. Furthermore, other results hints being preoccupation , inferiority
and as he might he be asserting masculinity and its sexuality he is still dependent on his mother
in which if pursued until older age may result in dependent personality disorder.
