 TWO-PARENT CONSENT REQUIREMENT FOR MINORS UNDER AGE 16

If NEITHER of the parents (or sole parent) is present, the person acting on their behalf must provide a
notarized Power of Attorney from ALL parents or legal guardian(s) authorizing him/her to act in their
capacity to apply for a U.S. passport for the child. The power of attorney must meet ALL the
requirements below. See sample on reverse side.

 Must have the name and date of birth of the child

 Must include the absent parent(s)’ address, phone number and email address (If they don’t
have an email address the letter must say “I do not have an email address.”)
 Must say ¨apply for a passport¨
 Must be signed by the absent parent before a notary public and be accompanied by a copy of
the valid, government issued ID with signature, that was presented to the notary public
 It is only valid for 90 days from the notarization date

If ONLY ONE parent is present, he/she must provide a notarized written consent from the absent parent
authorizing the execution of a passport application. It may be form DS-3053 available on-line at
http://www.state.gov/documents/organization/212243.pdf or any other statement that meets the
requirements listed above.

REQUERIMIENTO DE CONSENTIMIENTO DE AMBOS PADRES PARA MENORES DE 16 AÑOS

Si NINGUNO de los padres (o madre soltera) está presente, la persona que actúa en representación de
los padres debe presentar una carta poder notarizada en la cual los dos padres o tutores la autorizan
para firmar la solicitud y obtener el pasaporte del menor. La carta poder debe cumplir con TODOS los
requisitos abajo mencionados. Ver el ejemplo al reverso de la hoja.

 Debe tener nombre y fecha de nacimiento del menor

 Debe incluir dirección, teléfono y correo electrónico del padre/madre ausente (En caso de no
tener una dirección de correo electrónico, la carta debe decir: “No cuento con una dirección de
correo electrónico.”)
 Debe indicar que es para “obtener un pasaporte” o “solicitar un pasaporte ”
 Debe estar firmado por el padre ausente ante notario público y debe estar acompañado por una
copia de la identificación oficial vigente y con firma que se haya presentado ante el notario
 El documento tiene una validez de 90 días después de ser notarizado

Si SOLO UNO de los padres está presente, éste deberá traer el consentimiento escrito notarizado del
padre ausente autorizando el trámite de solicitud de pasaporte. Puede presentar la forma DS-3053,
disponible en línea en http://www.state.gov/documents/organization/212243.pdf o puede traer otro
documento que cumpla con los requisitos arriba mencionados.
