SURGICAL INSTRUMENTATION PROCESS

1. PLANNING STAGE:

1.1. Surgical Objective: (Make description)

It consists of tunneling the VSD towards
the single (neoaortic) truncal valve and the reconstruction of the RV outflow tract through a heterologous
homograft.

1.2. Anatomy and physiology: (Make graph and description).

Common trunk: This is the single artery that originates from

the right ventricle of the heart and carries both oxygenated and
deoxygenated blood.

Valve defect: Usually, in Truncus arteriosus, there is a single

valve that regulates the flow of blood from the right ventricle to
the common trunk. This valve may be abnormal or have
defects, affecting blood flow.

Coronary Arteries: The coronary arteries that supply blood to

the heart muscle may also be connected to the common trunk
instead of leading from the aorta as in normal anatomy.

This abnormality causes a mixture of oxygenated and

deoxygenated blood, leading to a significant decrease in the

body's oxygenation and can cause severe symptoms in affected babies.

 Extracorporeal circulation andcardioplegic cardiac arrest were used;The 5mm perimembranous VSD
wasclosed through the right atrium with 2U-shaped stitches and pericardialsupport. Next, the
pulmonary artery wasopened and the 3 lung commissures wereexposed with 3 Prolene 4/0
anchorpoints. The reconstruction of thepulmonary valve began with the anteriorleaflet, considering
arranging the serous(mesothelial) surface of the pericardiumof each cusp towards the
respectivesinuses. First, the neovelums were fixedto both commissures with 4/0 Proleneand then
implanted to the pulmonaryring with a continuous suture line. Theexit from extracorporeal circulation
andthe immediate postoperative period wereuneventful.
