Agreement Letter

This letter serves as proof of agreement between loan applicant and lender; stating that
Mr./Ms. _Wilson Moises__(name), an employee of Mediatrack since _16-08-2019 (date), with
residence at _Cristo rey, Capas, Taralac___(address), have agreed to avail a personal loan
amounting to __4000_____(pesos), from Kristella Muring, with 6% interest per month.

As agreed by both it will be paid upon _April 15 2023__(last date of installment fee), by
depositing to Kristella Muring’s BPI account a staggered amount of _1,120__ (in pesos, amt per
cut off), every 15th and 30th of the month until said date arrived or loaned amount has been
fully paid.

In the instance that the loan applicant will not be able to pay in full by the said date,
another 6% interest shall be added to that total pending loan amount.

In the instance that the loan applicant will not show for duty after loan has been
approved, the authorization letter shall take effect.
