Your internet service in my residence has been out for 2 weeks now and even after
countless follow-ups you have failed to restore your service which is a great
inconvenience as I have been working from home and am unable to do my job because
of this incident. I will no longer be needing your service.

Should there be anything else we need to settle or do aside from submitting this letter of
request, please let me know as soon as possible so I can act on it immediately.
