ACKNOWLEDGEMENT

This portion of your portfolio contains names of people or the institution you want

to acknowledge. You can enumerate the people who have been instrumental and

impactful in your Work Immersion experience. Remember to follow the prescribed

format. The Font style must be Arial in size 12. Indent the first sentence of your

paragraph. Justify your paragraph alignment and use double space.

This portion of the portfolio should only be one page.


ABOUT THE STUDENT OBSERVER

This part of the portfolio will allow your evaluator to get to know you. You need to

provide basic details about yourself, such as your date of birth, the name of your

parents, and the like. But more importantly, include details about yourself relevant to the
work immersion program, such as your aspirations and motivations. Below are some

questions that might help you fulfill this task but remember that these are merely guiding

questions. 1. What is your dream job or career?

2. What motivates you to aspire for that job or career? Is there a significant person that

helped you in your decision?

3. What characteristics or traits do you possess that you think will be of great help to you

in fulfilling your goals?

Be sure that you observe proper margins and indentation. Always use the

prescribed formatting styles. This page should be at least two pages but should be at

most four.

ABOUT THE PARTNER AGENCY

You are expected to give an overview of the partner agency you were assigned

to. You can use the following as a guide in writing this portion:

a. Brief historical background of the business/ nature of the business b. Description

of the products/services of the company (what kind of business and products they

have)

c. Target clientele (who are the clients or customers of the company)

d. Cite some company rules and regulations

e. Safety/ production/ maintenance/ quality control/ quality assurance/ customer

satisfaction/ housekeeping/ hygiene.


If you must take something from the internet, such as the company’s website, make

sure that you paraphrase and acknowledge the source. If you cannot paraphrase, don’t

forget to use quotation marks.

This portion of your portfolio will give your evaluator an idea of what your partner

agency is like and will serve as a reference point for the other narratives about your

agency. Your discussion should reflect your knowledge about the company/agency you

were assigned.

PARTNER AGENCY’S MISSION, VISION, GOALS, AND OBJECTIVES

You can type the mission, vision, goals, and objectives or include a photo of the

following. Don’t forget to include headings or captions as labels. If you choose to obtain

their mission and vision, goals, and objectives from the web, cite your source.

If your partner agency, say, the Dept. of Public Works and Highways, belongs to

a larger group, agency, or organization, you may use their general mission, vision,

goals, and objectives.

Lastly, if you were assigned to an agency that does not have any, kindly write on

this portion that the agency has no existing mission, vision, goals, or objectives. Be sure

that you observe proper margins, indentation, and formatting styles.

PARTNER AGENCY’S ORGANIZATIONAL STRUCTURE

For this part of the portfolio, you are expected to show the organizational

structure of the partner agency you were assigned to. You can either put a photo of the

organizational structure or re-create them using your word processor features. However,

you must identify which department you reported directly to during your work immersion.
See the sample below as a guide. If you were assigned multiple sections/departments,

choose only two (2) departments to showcase the existing organizational structure.

Showing which part of the organizational structure you belong to allows your evaluator to

have an idea of the scope of experiences and exposure, you had that would be reflected

in your narrative reports.

You may include a brief paragraph to explain the organizational structure.

Remember to follow the formatting styles and guidelines.

PREPARATION FOR MY WORK IMMERSION

In this portion of your portfolio, you are expected to narrate and recount what

preparations you underwent before the start of your Work Immersion Program. For

example, describe the experience of getting your first NBI Clearance, Barangay

Clearance, or Medical Certificate.

If you did not have to do any of those above, you could narrate what preparations

you had to make for your work immersion. Remember to follow the formatting styles and

guidelines.

FIRST IMPRESSION

In this portfolio portion, you must narrate your first impressions of your partner

agency. The following are questions that can help you fill this part. The questions and

statements below are merely prompts to help you, but you can include other relevant

responses to the task.

page must be at least two pages but must be at most five pages.

a. What was your initial reaction when you found out what agency you will be

immersed in?

b. Were you excited when you found out what your partner agency is? Why or why

not?

c. What expectations or apprehensions did you have upon knowing your partner

agency? What reason/s do you have for feeling as such?

d. Upon your arrival at the partner agency, what notable experience/s did you have?

(If there are any.)


e. Describe your initial impression during your workplace orientation or the

preliminary visit.

immersion. You can narrate the assigned tasks and the insights you have learned from

reflections.
