EXPLANATORY LEAFLET FOR THE PREVENTION OF WORK ACCIDENTS

INCIDENT - PAINT CABIN DOOR FALL

On October 27th 2022, at 9:20 am, the access door to the blasting and painting cabin collapsed on
one side and fell to the floor from a height of approximately 15m. At the moment, there were
activities and people inside the cabin.

Immediate Cause:

• It was identified that there was a collapse

of the steel cable of the high-power system
and emergency brake system failure door.

Basic Cause:

• Non-existence of the project/technical

drawing for door assembly (structure +
drive system).

Corrective action:

• First check the existence of the

project/technical drawing for
fabrication/installation of any equipment;

• Elaboration and installation of the anti-fall

retractable system for the door;

• Inclusion of blasting cabin door

equipment in the preventive maintenance
program.
