HEALTH WORKER of Barangay West Dirita, San Antonio, Zambales, effective
upon assumption, with authority to discharge the functions and duties vested in
the said office in accordance with laws.
This appointment shall be enforced and valid sooner until revoked by the
undersigned subject to existing laws, rules and regulations.
