    At present, I am an applicant for Enlistment in the Philippine Coast Guard (PCG). As


    part of the PCG’s security protocol, the PCG personnel will conduct Background Investigation
    (BI), including scholastic record check, to verify the authenticity of the documents from the
    university/college that I have submitted to the said agency.

    In this regard, I am executing this authorization to the bearer who is a member of the
    PCG, to verify the authenticity of my scholastic records. Likewise attached is the receipt of
    payment dated that I previously obtained from the ________ from the
    authentication of the said documents.
