Executive Summary
The Health Resources and Services Availability Monitoring System (HeRAMS) collected data from 1,098
public healthcare facilities in Ukraine from November 2022 to January 20231 in the 10 oblasts of Ukraine
most affected by the war. Due to the unstable security situation in Khersonska oblast, data collection was
initially facilitated by a third-party monitoring group specializing in health assessments. However, following
the baseline data collection, the Khersonska oblast health department took responsibility for updating the
data and expanding the data collection.
In parallel with the ongoing update of the data in the most affected oblasts, data was collected from late
March to May 2023 for the remaining 13 oblasts and Kyiv city with the exception of Luhanska oblast and
Avtonomna Respublika Krym as well as areas of Donetska, Khersonska, and Zaporizka oblasts under the
temporary military control of the Russian Federation. In total data from 2,333 public healthcare facilities was
collected.
Information was collected on the functionality of healthcare facilities, as well as the extent of damages, basic
amenities, and service availability in the following domains:
• Trauma and general services
• Child health and nutrition
• Communicable diseases
• Sexual and reproductive health
• Maternal and newborn care
• Non-communicable diseases (NCDs)
• Mental health and psychosocial support (MHPSS)
The data collected by HeRAMS showed that healthcare facilities are functioning, and services related to
primary healthcare are widely available across the country. However, more specialized services such as
chemotherapy, mammography, and the multidrug-resistant tuberculosis diagnostics and treatment are inter-
rupted which is largely due to the lack of personnel and medical equipment.
Service disruptions are most visible in oblasts impacted by the conflict. Overall, 96% of the assessed health-
care facilities are fully functioning.
However, in Donetska oblast around a third (27%) of healthcare facilities are non-functioning, while another
third (36%) are partially functional due to a lack of security and structural damage to the healthcare facil-
ities. Additionally, half (50%) of all facilities in Donetska oblast are partially damaged and 10% have been
destroyed since the beginning of the full-scale war on 24 February 2022.
In Khersonska oblast, 43% of healthcare facilities are functioning, even though 71% sustained partial or total
damage to their building, and 58% of health facilities sustained partial or total damage to their equipment.
The condition of medical equipment is particularly concerning in Kharkivska and Donetska oblasts, where
23% and 55% of facilities, respectively, report fully or partially damaged medical equipment.
In all assessed oblasts, trauma and general services are widely available, but more specialized medical ser-
vices are interrupted due to lack of personnel and medical supplies and equipment.

Less than 1% of healthcare facilities report disruption to non-communicable diseases clinics.
However, almost two-thirds (69%) of facilities in Khersonska oblast, more than half (55%) of healthcare
facilities in Donetska oblast, and over a third (40%) of facilities in Sumska oblast do not provide skilled care
during childbirth. The situation in these oblasts is similar for basic emergency obstetric care, caused primar-
ily by the lack of trained personnel.
Additionally, specialized diagnostics such as mammography screening have been disrupted in the majority
of oblasts with the exception of Vinnytska oblast. The proportion of facilities that no longer offer mammog-
raphy screenings ranges from 11% in Zaporizka to 66.7% in Ivano-Frankivska oblast. The situation is com-
parable for cancer diagnostics such as cystoscopies, bronchoscopies, and prostate biopsies. The primary
reasons for the unavailability are, once more, lack of specialized medical equipment and personnel.
In summary, the all-country baseline data collection has shown that health services are generally available
across Ukraine.
In areas that were not directly affected by military action, most services are widely available across all do-
mains. The infrastructure and basic amenities are also in good condition, compared to areas that were on
the frontline of the conflict. This is because these areas were not subject to the same level of destruction
and damage as the frontline areas.
In conflict-affected areas, including those that the Ukraine Government has recently regained control of, key
services are not available due to damages to buildings, lack of medical equipment, and lack of specialized
personnel.
If diagnostic services such as mammography and biopsy and cancer treatment continue to be disrupted,
this will almost certainly have a negative impact on public health, as early detection of noncommunicable
diseases and subsequent treatments will be delayed. The unavailability of basic emergency obstetric care in
certain oblasts is a further cause of concern and poses a significant threat to maternal health.

Methodology
Since November 2022, the Health Resources and Services Availability Monitoring System (HeRAMS) has
collected data from 2,333 healthcare facilities, which represents virtually all public healthcare facilities in
all oblasts of Ukraine with the exception of Luhanska oblast and Avtonomna Respublika Krym, including
healthcare facilities along the frontline. However, if no data were available for the areas of Donetska, Kher-
sonska, and Zaporizka oblasts under the temporary military control of the Russian Federation, they were
excluded from analysis. Private healthcare facilities were excluded from the data collection because they
represent only a small fraction of the healthcare facilities across Ukraine. Instead, the data collection focused
on public healthcare facilities.
Multiple data collection rounds, determined geographically, were conducted: Round 1 data collection
began in November 2022 and ended in February 2023, in nine most conflict-affected oblasts: Odeska,
Mykolaivska, Zaporizka, Kharkivska, Donetska, Sumska, Chernihivska, Kyivska, and Dnipropetrovska oblasts.
In all oblasts but Khersonska oblast, subnational focal points nominated by the oblasts’ health departments
were initially engaged in data collection. These focal points attended a two-day in-person training workshop
to ensure familiarity with the questionnaire and ensure a common understanding of key concepts such as
‘accessibility’ and ‘functionality’ of a healthcare facility.
Due to the unstable security situation in Khersonska oblast, data collection was initially facilitated by a
third-party monitoring group specializing in health assessments. However, following the baseline data col-
lection, the Khersonska oblast health department took responsibility for updating the data and expanding
the data collection.
In March 2023, data collection began in the 13 remaining oblasts of Ukraine and Kyiv city by subnational fo-
cal points who had been nominated by the oblasts or Kyiv city health departments (Zhytomyrska, Vinnytska,
Cherkaska, Poltavska, Khmelnytska, Rivnenska, Kirovohradska, Volynska, Lvivska, Ternopilska, Chernivetska,
Ivano-Frankivska, Zakarpatska oblasts, and Kyiv City). The focal points had completed a standard two-day,
in-person training prior to beginning data collection.
The questionnaire and training materials were adapted to the local context and translated into Ukrainian.
All healthcare facilities included in the master healthcare facility list provided by the National Health Ser-
vice of Ukraine were contacted. The overall response rate at the end of Round 1 was over 99%. For Round
2, the dataset was completed in accordance with the initial master healthcare facility list. The most recent
data update for Round 1 was centrally initiated in mid-April, ensuring that data from the Round 1 regions are
chronologically comparable with the data from other oblasts.
Basic cleaning and logic checks were performed on the raw data to meet quality standards. Additional qual-
ity assurance was piloted thought through a third-party on-site data verification of 70 healthcare facilities
located in a 10 to 50 kilometers zone from either the state border or the frontline in Donetska, Kharkivska, and
Zaporizka oblasts through the third-party monitoring group. The data in the HeRAMS platform varied only
in 6.37% of cases from the observations of the monitoring group. In half of the cases the variations occurred
due to context and situation changes.

Conclusions
The conclusions of this report were generated through an interpretation workshop held on 7-9 of June 2023.
The workshop brought together 70 representatives from 23 oblasts and Kyiv city, including officers and
management from oblast Health Departments, representatives from oblast Medical Information Centers,
subnational Public Health Centers, and healthcare facilities.
Building and equipment damage: The most affected oblasts were Donetska, Kharkivska, and Khersonska
mainly due to armed conflict. In less affected areas, healthcare facilities sustain damage due to insufficient
maintenance. Because the network of emergency medical care centers is decentralized, and damage to
ambulances is not reported in the HeRAMS platform, emergency medical care centers reported the lowest
percentage of damage.
Functionality and accessibility: It was noted by the participants that despite the damage to healthcare
facilities, indicators of functionality and accessibility are quite high. Frontline areas (North-Eastern, Eastern,
South-Eastern, and Southern parts of Ukraine) had the lowest availability and functionality, followed by Cen-
tral oblasts and North-Western oblasts, and finally Western oblasts, which had the highest availability and
functionality. Lack of security was mentioned as the primary cause of inaccessibility and ineffectiveness.
However, shortage of medical personnel and physical obstacles (e.g., distance, damaged roads) were also
reported and, according to the participants of the interpretation workshop, additional information is required
to determine which critical infrastructure is damaged, which access barriers exist and which medical per-
sonnel is required. The majority of healthcare facilities did not report financial obstacles. In terms of types
of healthcare facilities, emergency medical centers and multiprofile healthcare facilities had the highest
functionality and accessibility indicators.
Availability of energy supply, cold chain, heating, water supply and their sources: During the workshop
participants discussed that the availability of energy supply, cold chain, heating, water supply and their
sources are interconnected. They are mostly reported to be received from the public network, which makes
healthcare facilities dependent on their functioning. For example, healthcare facilities need to maintain a
cold chain to store vaccines and other medical supplies. Participants also highlighted that there is sufficient
availability of energy and heating. However, there are reports of lack of financial resources and equipment in
cases where basic amenities are impeded. This can lead to disruptions in health care delivery.
Medical waste disposal: Participants highlighted that there is a persistent nationwide problem with waste
disposal, including medical waste. This is why no significant differences in the availability of waste disposal
(including acute and infectious waste) were reported between the oblasts. Participants expressed concerns
about the suboptimal ways of waste disposal that were reported. In the discussion, it was mentioned that
more than 50% of facilities dispose of waste off-site, delegating this function to the external vendors. The
main obstacles to on-site waste disposal are the lack of equipment and funding.

Service domains: During the overview of service domains, it was noted that the availability of services
depends on proximity to the frontline. The main reasons for inaccessibility, regardless of the region, are the
lack of personnel and equipment. The participants speculated that the high percentage of personnel short-
ages was associated with the outflow of qualified workers abroad (brain drain). The highest percentage of
non-availability and partial availability was presented by reproductive health, maternal and newborn care,
noncommunicable diseases and mental health domains. In the Central and Western oblasts, the common
reason for service unavailability was lack of finances. In the meantime, throughout the country, the leading
reason for limited availability of mental and sexual reproductive health services was lack of training and
capacity building.
Basic emergency care, advanced care based on syndromes, emergency inpatient care: During the
discussion of the indicators for basic emergency care, advanced care based on syndromes, and emergency
inpatient care, it was noted that conflict-affected oblasts reported the lowest availability of these services.
The group discussed the differences in the impediments to these services i.e., the common barrier was lack
of equipment, however for the emergency inpatient care one of the leading barriers was lack of personnel.
It was emphasized that inadequate availability of basic emergency care is unacceptable, even though the
lowest availability was reported by primary healthcare institutions.
Treatment of minor injuries: In terms of complete treatment of minor injuries, it was concluded that it is fully
available in the healthcare facilities mostly when specialized personnel is available (e.g., surgeons, trauma-
tologists). The reported barriers to service delivery were lack of medical equipment and capacity building.
Emergency and planned surgical operations and orthopedic/trauma department: Regional differences
were discussed, and the principal barrier to service delivery in the conflict-affected oblasts was shortages of
personnel, whereas in other oblasts the principal barrier was lack of financial resources.
Outpatient services of primary medical care, outpatient department specialized care, and outpatient
care for mental disorders: Although it was discussed that the availability of outpatient services is quite
high, the data indicate that only in 12% cases outpatient care for mental disorders is available at the primary
healthcare level. The main barrier was lack of training, particularly at the primary level, and lack of person-
nel in all types of healthcare facilities. Whereas lack of equipment was the main obstacle to the delivery of
outpatient specialized care.
Noncommunicable Diseases (NCDs): The group discussed the widespread availability of services for hy-
pertension, diabetes, asthma, and COPD, while the availability of services for cancer diagnosis and treatment
services was low. The services for acute myocardial infarction and acute cerebral stroke were deemed to be
available in all fully qualified and equipped healthcare facilities. The group hypothesized that several health-
care facilities reported obstacles to providing services for myocardial infarction and acute cerebral stroke,
such as lack of qualified personnel and equipment.
The availability of services for diabetes, asthma, and COPD varied geographically, with the lowest availability
in the most conflict-affected oblasts mainly due to lack of personnel. Similarly, there were no significant
differences between oblasts in terms of the accessibility of cancer treatments and diagnostics. Participants
in the interpretation workshop hypothesized that this could be due to the fact that cancer services are pri-
marily offered in specialized healthcare facilities (monoprofile and multiprofile healthcare facilities). Lack
of diagnostic equipment was cited as the most prevalent barrier to cancer diagnosis. In this regard, it was
stated that there were no non-instrumental cancer screening indicators at the primary healthcare level. Ad-
ditionally, the most frequently cited obstacle to cancer treatment was lack of personnel.

Furthermore, it was highlighted that the humanitarian aid did not meet the needs of patients with hyperten-
sion, diabetes, asthma, and COPD due to the short remaining shelf-life of the provided medications and the
fact that the delivered pharmaceuticals were not typical of Ukrainian prescription practices.
Moreover, the group expressed doubt that lack of personnel and medical equipment in primary healthcare
facilities to the hypertension diagnosis and treatment.
Sexual and reproductive health (maternal and newborn care): It was discussed that antenatal care,
postnatal care, and prevention of mother-to-child transmission of HIV (PMTCT) were insufficiently provided
in healthcare facilities, including primary healthcare facilities. Lack of personnel and training were cited as
obstacles to these services. Donetsk and Kherson regions exhibited the lowest availability. Even though the
state and NGOs/INGOs provide a substantial amount of resources for HIV-related services, the group found
it concerning that there is a lack of medical supplies for PMTCT.
The availability of skilled care during childbirth, basic emergency obstetric care, and comprehensive emer-
gency obstetric was the lowest in the most conflict-affected oblasts, even though the services are available
at the national level. Personnel and equipment shortages were reported for all childbirth-related services.
The group questioned the reported availability of skilled birth attendants at several primary health care
facilities in Ukraine.
Sexual Violence: The clinical management of rape survivors, including first psychological aid, emergency
contraception, and post-contact prevention, was reported to be extremely limited in availability at the level of
primary healthcare, regardless of the oblast. Common barriers cited for these services were lack of person-
nel and training, whereas for emergency contraception, the primary barriers were lack of medical supplies
and financial resources. Concerning immunization, the group did not reach a consensus on whether or not
88% of Expanded Program on Immunization availability at the primary healthcare level was sufficient, nor
did they agree on whether or not there were differences in the program between oblasts. Again, the group
questioned the accuracy of the obstacles reported by the primary healthcare facilities, specifically the lack
of personnel and lack of equipment.
Epidemic and emergency preparedness: Despite the fact that epidemic and emergency preparedness
was generally available across healthcare facilities with limited availability of emergency capacities cited
lack of personnel and medical equipment as the primary obstacles. Local priority infectious diseases, and
the management of patients with complex or complicated course of infectious diseases were deemed wide-
ly available across, but even less so in conflict-affected oblasts. In instances in which service delivery was
unavailable, lack of personnel was cited as the primary obstacle.
HIV and Tuberculosis (TB): Due to the limited availability of these services at the primary healthcare level,
the group could not agree on whether the availability of HIV testing, appropriate counseling, pre-exposure
prophylaxis, and ART could be deemed high. For example, the majority of ART was provided by specialized
healthcare facilities. Due to the substantial external support for HIV-related services, the group was once
more alarmed by the data indicating lack of medical supplies for ART provision.
In conflict-affected oblasts, tuberculosis and multidrug-resistant tuberculosis services are less accessible. In
all the participating oblasts, the same obstacles were cited, namely lack of personnel and equipment. During
the course of tuberculosis treatment, the patient has a choice between primary care and a specialized insti-
tution. Therefore, the group suggested that it would be advantageous to differentiate the data on barriers by

the types of healthcare facilities in order to determine which personnel were required (general physicians
or tuberculosis specialists).

Interpretation Guide
Indicator status
Arc charts
For each indicator, an arc chart provides an overview of the overall
status (i.e. functionality, availability, sufficiency, etc.), hereafter referred

Reasons of unavailability
If an indicator was not or only partially available, main reasons of unavailability (i.e. causes of damage, reasons for
non-functionality, etc.) were collected. Similarly, indicators assessing availability and sufficiency of basic amenities may
have a sub-question gathering additional information on the type of amenity available. Alike reasons of unavailability,
types of amenities are only evaluated if the amenity was at least partially available. For simplicity reasons, causes of
damage, non-functionality and inaccessibility, reasons of unavailability, types of basic amenities, and type of support
provided by partners are hereafter commonly referred to as “reasons”.
