Create an authentic assessment that makes the learners demonstrate practices for building and
maintaining healthy school and community environments (Note: This is the performance
standards.)
1. Write the assessment in paragraph form using the format below.

You are a member of a science organization. The community where the school is located is suffering from
improper waste segregation and clogging of water canals. You are tasked to interview the residents about
their concerns and suggestions to fix the problem. You will create a vlog or documentary that contains the
information you gathered from the residents. Also, demonstrate what would be the possible actions to
be undertaken to solve the problem. You will present this to your Science teacher and Science Club
Adviser. Your output will be judged according to the following criteria: clarity of speech, the accuracy of
the information to solve the situation, the technical appeal of the video, communication, and purpose of
the output, and delivery and narration.
