Pt is a *** who presents for follow up of ADHD.

Parents report good control of symptoms


during the time that the medicine is in his system. Parents believe that a single dose of
the current medicine last approximately *** hours. At home, the medicine is given
occasionally on the weekends and summer. Most recent teacher feedback is positive.
Pt has had good grades and has not been disruptive in class. Timing of the medicine
appears to be appropriate throughout the entire day.

Pt has had a good appetite, especially when the medicine is not on board.
Pt has been sleeping well.
No comorbidities have been noticed including depression, anxiety, or aggression.
No rebound irritability has been noticed at home or school.

Assessment:
Attention deficit hyperactivity disorder (ADHD), combined type - F90.2

Plan:

Goals for treatment discussed, all questions and concerns were answered. Any
concerns for changes in efficacy, school performance, behavior, mood swings, or
development of new medical problems should be brought to my attention.

Follow up in 3 months, sooner for problems or concerns.

May use telemedicine for follow-up visit if desired.
