Electronic Records Disclosure

To complete an application for an account and related products and services offered by UMB Bank, n.a.
(“UMB”) through UMB’s website, you must first review and accept this Electronic Records Disclosure (the
“Consent”).

By clicking on the checkbox that confirms that you have reviewed and consent to this Consent, you agree
to receive various agreements, disclosures, communications, and notices related to the account you are
applying to open on UMB’s website, and any related products and services, electronically, rather than in
paper form. If you do not choose to consent to receipt of Electronic Records (defined below), you will be
unable to complete an application for an account through UMB’s website. Instead, you will need to visit a
UMB branch to apply to establish an account.

Scope of Agreements and Communications to be Provided in Electronic Form

If you consent, you will receive electronic records that relate to your application for accounts, products
and services (the “Electronic Records”) including, but not limited to:
(1) This Consent and any changes to it;
(2) Product and service agreements and disclosures;
(3) Disclosures under the Electronic Funds Transfer Act, the Truth in Savings Act, the Expedited
Funds Availability Act, and the Gramm-Leach-Bliley Act, and any other disclosures or notices
that may be required under applicable federal or state laws and regulations;
(4) Privacy policies and all other information related to the products and services; and
(5) Transfer authorizations, confirmations, alerts and other communications, including but not
limited to electronic fund transfer authorizations, responses to notices of error allegation
involving electronic funds transfers, and related consumer disclosures required by law.

This Consent does not include an agreement on your part to receive your account statements or your tax
statements in electronic form. You will have the option to elect to receive those documents electronically
once you log into UMB’s digital banking platform. This Consent is limited to the account for which you are
currently applying. This Consent does not revoke or alter any consent that you may have previously
given to us to send your account statements or other information to you solely via electronic means.

We may provide Electronic Records to you in any of the following ways: (1) by providing a link or other

with instructions and access to a PDF file containing the Electronic Records; (3) by email message or

be in the future made available to you. When we provide Electronic Records in any of those ways, we will
have delivered the Electronic Records to you, whether or not you choose to access the specific Electronic
Records we are sending you at that time, to save them, or to print them.

Depending on the type of account you have, there may be a charge for requesting paper copies of other
records, such as account statements.

How to Withdraw Your Consent

You may withdraw your consent to receive Electronic Records or change the email address you have on
file with UMB by calling 1.877.514.0048, Monday – Friday, 9 a.m. – 5 p.m. CT, or Saturday, 9 a.m. – noon
CT. Any withdrawal of your consent to receive Electronic Records will be effective only after we have had
a reasonable period to process your withdrawal and will not impact our ability to send you electronic
communications, to the extent not otherwise prohibited by applicable law. If you withdraw your consent to
receive Electronic Records while your application is pending, your application will be withdrawn.

Communications in Writing

All communications in either electronic or paper format from us to you will be considered "in writing." You
should print or download a copy of this Consent in writing, along with any other Electronic Record that is
important to you. We reserve the right, but assume no obligation, to provide a paper (instead of
electronic) copy of any communication that you have authorized us to provide electronically. If we choose
to provide a communication in paper form, that will not revoke your authorization to receive Electronic
Records, as provided in this Consent.

Federal Law

You acknowledge and agree that your consent to Electronic Records is being provided in connection with
a transaction affecting interstate commerce that is subject to the federal Electronic Signatures in Global
and National Commerce Act (the "Act"), and that you and UMB intend that the Act apply to the fullest
extent possible to validate the ability to conduct business with you by electronic means.

Joint Accountholders

Each joint accountholder may provide contact information for purposes of receiving electronic

2
communications (such as an e-mail address) in connection with each joint accountholder's consent to
receive Electronic Records. Delivery of an Electronic Record to any joint accountholder shall constitute
delivery to all joint accountholders, unless you have expressly revoked this Consent.

Acknowledging You’re Access and Consent to Receive Materials Electronically

Electronic Records Disclosure, you agree that:

you can print the Electronic Records or save or send the Electronic Records to a place where
you can access them in the future;
