This is to inform you that the Senior High School Department have been provided
with a laptop for official work having the serial number: SHS-Caregiving-REG4A-
2018-05-Computer-001 by the DepEd Regional Office. I am most humbly writing
this letter in order to inform you that the above-mentioned laptop which is provided
to our department is not working already. The issues with the provided laptop such
as screen with two-lines, no hard drive, broken charger, default settings
not working and black screen system.

Therefore, may I request that kindly look into this matter and provide us with
