JUDICIAL AFFIDAVIT OF COMPLAINT

PRELIMINARIES:

a. This Affidavit is executed by Mr. Kent H Yu, 35 years old, single, resident of Purok-6,
Brgy, Kinamlutan, Butuan City.

b. This affidavit is taken by Pcpl Arnold Caliso, Investigator, inside Butuan City
Police Station 3, Langihan Butuan City in English dialect and translated into visayan
dialect known to her and understood to the affiant.

c. Mr. Kent Dunas answered the questions asked of her, fully conscious that
she does so under oath and that he may face criminal liability for false testimony or
perjury.

QUESTIONS AND ANSWERS

Question 1. : WHAT IS YOUR PURPOSE IN EXECUTING THIS AFFIDAVIT?

Answer : I am executing this affidavit to stand as complainant to file a formal

complaint against Rex Entero, 34 years old, male , single and resident of Purok 6,
BRGY Kinamlutan Butuan City for the crime of Robbery

A : the suspect forcefully and intimidate me with a knife to give my cellphone and
wallet and warned me not to shout for help so that he will not kill me

A : On February 24, 2027, at about 10:35 PM in P-6, BGRY Kinamlutan , Butuan

City. I was sitting in the waiting shed near at my house with my friend Jon Jones , to find
some signal because I was playing Mobile legends and suddenly a Motorcycle XRM
blue colored stop by and I thought it was one of the resident and yes he is a person
whom I know Rex Entero, 34 years old, male , single and resident of Purok 6, BRGY>
Kinamlutan Butuan City. We thought he was gonna hangout with us but suddenly he
pulls a knife and tried to intimidate me saying “give me your phone and wallet or else I
will kill you”
: Niadtong Pebrero 24, 2027, sa mga 10:35 PM sa P-6, BGRY Kinamlutan ,
Butuan City. Naglingkod ko sa waiting shed duol sa akong balay uban sa akong amigo
nga si Jon Jones, para mangita ug signal kay nagduwa ko ug Mobile legends ug kalit
lang naay nihunong nga Motorsiklo XRM nga kolor blue ug abi nakog usa kadto sa
residente ug oo siya usa tawo nga akong nailhan nga si Rex Entero, 34 anyos, lalaki ,
ulitawo ug residente sa Purok 6, BRGY> Kinamlutan Butuan City. Nagtuo mi nga mag-
hangout mi pero kalit lang siyang mibira ug kutsilyo ug misulay sa paghadlok nako nga
nag-ingon nga "hatagi ko sa imong telepono ug pitaka kay kon dili patyon ko ikaw"

6. : WHAT HAPPENED NEXT?

A :after that I got scared and I did not even can do any step to runaway while my
friend Jon Jones ran. And I did give my phone and my wallet and he went away with his
motorcycle

JOINT AFFIDAVIT OF ARREST

We, the UNDERSIGNED POLICE OFFICERS, both members of the Philippine
National Police, presently assigned at the Butuan city police station, do hereby depose
and state THAT;
1. We are among the members of the Regional Operation Division, PRO13 ,
who conducted entrapment operation at FLOR-Al mansion J. C Aquino Avenue,8600

2. Said operation stemmed from the complaint of Kent H. Yu for the crime of robbery
which referred to us by unknown informant calls against Rex Entero, 34 years old, male ,
single and resident of Purok 6, BRGY Kinamlutan Butuan City for the crime of Robbery
3. On the complaint dated February 25 2027, on the crime of Robbery
4. Upon receipt of their said complaint, our office planned out an entrapment operation against
the suspect. At about 1:30 pm in the afternoon of 27 of February 2027, the undersigned together with
the complainant proceeded at the entrapment of the suspect at one of the room in at FLOR-Al
mansion J. C Aquino Avenue,8600 Butuan City

5. Upon arrival thereat, complainants was check-in in room 502 sleeping, while the
authorities knock the door no one was able to response and the door was forcefully
opened and the suspect was caught trying to ran away and immediately handcuffed.
The arrested suspect inform of his constitutional rights in visayan known and
understood by him

6. Seized and recovered in the possession and control of the arrested person
were the following, to wit:
1. The Cellphone Iphone 7plus 128gb

2. wallet of the complianant

3. kitchen knife

4. 2gram of ectasy and a pack of weed

7. Suspect together with the seized and recovered evidence was brought to the PNP Regional
Crime Laboratory Office in PNP Camp Rafael Rodriguez, Butuan City for booking and proper
disposition. Likewise, the arrested person was apprised of their rights as provided for under the
Constitution of the Philippines
 We executed this joint affidavit to attest the truthfulness of the foregoing facts and to support
the filing of Criminal Cases complaint against Rex Entero, 34 years old, male , single and
resident of Purok 6, BRGY Kinamlutan Butuan City for the crime of Robbery .

AFFIDAVIT OF WITNESS
I, Jon Jones, 27 years old, male, single and Resident of P-6 P-6, Brgy.Kinamlutan,
Butuan City:
1. I was present and personally saw the accuse against Rex Entero, 34 years
old, male , single and resident of Purok 6, BRGY Kinamlutan Butuan City against
Mr. Kent Dunas
2. At around 10:40PM , February 25, 2027 I was in the outside of my house at
Purok 7 Brgy Kinamlutan Butuan City in waiting shed looking for a great signal to
my phone so that I can play well in mobile game
3. Suddenly I saw Mr. Kent Dunas being harass by a man holding a knife saying
give your cellphone and wallet or else I will kill you and I ran off because I was
scared

I executed this affidavit to attest the truthfulness of the foregoing facts and to support
the filing of criminal cases against Rex Entero, for violations of robbery.
