Petrokimia Gresik, East Java, Indonesia from January 9* to January 10", 2014. ‘The HAZOP Studies Technique proceeded in this report is based on publication entitled Guidelines for Hazard Evaluation Procedures issued by American Institute of Chemical Engineers (NY). ‘The HAZOP studies are a systematic review of a defined system in order to identify potential process hhazards and operability problems by examining deviation from design intent. In the HAZOP studies, potential process hazards and operability problem that may occur during normal ‘operation shall be identified After the HAZOP study meeting concluded, the HAZOP chairman issued recommendation report as a ‘guideline to rectify identified potential hazard. ‘The Ammonia Tank Capacity 20,000 MT Double Wall Project Engineering Manager is responsible to review all recommendation from the result of HAZOP Study Meeting. 2. Seope and Study Approach ‘The HAZOP study reviewed the om-plot P&ID's listed in Attachment-1. These P&ID’s had previously been prepared by PT Rekayasa Industri ‘The study team member consisted of PT. Petrokimia Gresik (owner) and PT. Rekayasa Industri (EPC). In order to maintain the independence of the HAZOP Study Team, this team is separate ftom the Project Design Team, led by HAZOP Chairman and assisted by a Secretary (HAZOP Scribber) HAZOP Study methodology is used to identify potential hazards on the operation of process units. In order ‘to improve post study safety and reliability margins, it is essential that a clear set of the proposed recommendations to be defined and the responsibility for any further action be assigned to the appropriate section/department. To assist the HAZOP Study methodology, the study nodes were selected by the HAZOP Chairman in conjunction with the appropriate Process Leader from PT. Petrokimia Gresik and PT. Rekayasa Industri at Biro Diklat PT. Petrokimia Gresik, East Java, Indonesia. ‘As a general impression of study, the Plant is well designed and the attendance knows well the operation ofthe Plant, both the similar existing plant and the newly designed plant. ‘As a result of HAZOP Study meeting, there are some recommendations as shown in the list have been recorded. Recommendations are classified into the following: a, Request of addi ‘These recommendations request provisions for safety or operability improvements to be newly added in the design. This type of recommendations are listed at Attachment-4 and Attachment. '. Matters to be confirmed or investigated outside the meeting ‘These recommendations request confirmation or verification of the matters on which design basis al provisions or detailed information are not available or not clear at the meeting, This type of recommendations are listed at Attachment-4 and Attachment-5. ‘These HAZOP Study result also can be used as reference for preparation of SOP (Standard Operation Procedure) by PT. Rekayasa Industri such as start up, shut down and commissioning procedure. PT RERAYASA INDUSTRI Rev HAZARD AND OPERABILITY (HAZOP) STUDY REPORT P-41-32-800-15-001 ‘were used to evaluate the Full process requirement for the Ammonia Tank Capacity 20,000 MT Double ‘Wall Project using the HAZOP study technique by the HAZOP Team Leader. ‘The actual study nodes during the HAZOP review are shown in Attachment-2, These study nodes are recorded on the master copy of HAZOP P&ID's (Attachment-6), ‘The participants on each study sessions are recorded by the Daily Attendance Record (Attachment-3), Due to necessity to perform design change on instrument air and plant air system, this specific node (Node-14) was discussed only for one deviation (Deviation-13) which addressed effects resulting from total loss of instrument air to new Ammonia Tank facility. Detailed HAZOP study for this node may be done at later date whenever necessary. 3, Study Result ‘The HAZOP Study conclusions were recorded in the worksheet using the program PHA Ver 5.0 Dyadem Inc. The worksheet is shown on Atachment-4. During HAZOP Study, a total of 66 recommendations were raised.
