We write to you to express our intent and interest to establish the classification of MED
CONSULT DIAGNOSTICS INC., as a Primary Clinical Laboratory located in 238 Pablo
Ocampo Sr. Ext., San Antonio, Makati City, 1203, Metro Manila, NCR.

Previously or in 2002, LAGMAN DIAGNOSTIC CENTER located at 1658 Caton St., Cor.
Archimedes St., Brgy. La Paz, Makati City was established, of which I am the sole owner.
Lagman Diagnostic Center is classified as a Primary Clinic. Meanwhile, since it started its
operations, we have been able to provide high quality, fast and accurate, economically priced
medical services to low, medium, and high-income families as well as small, medium, and large
companies.

However, with the current pandemic, similar minded individuals have decided that it was time to
expand and provide additional medical services to the public. Hence, the birth of MED
CONSULT DIAGNOSTICS INC. At present, I am the Medical Director of Lagman Diagnostic
Center. With the bigger and more modern facilities of MED CONSULT DIAGNOSTICS INC.,
we will be able to provide a wider range of medical services to our growing clients. Given my
tenure in the medical services field and the new modern facilities we have established, we are
more that capable and qualified to upgrade our classification to a Primary Care Facility.
