WEEKLY HEADS-UP AND ANNOUNCEMENTS

From the Office of Registration and Records:


1. Submission of Incomplete Entrance Credentials and
Requirements:
For those who have incomplete entrance credentials and
requirements, please coordinate with your respective class
advisers. Your class advisers will announce your lacking
documents for you to comply. Everyone is given until October
16, 2023 to comply. Failure to comply may lead to delays and
issues.

From the Office of Guidance Services:


2. Accomplishment of Student Individual Inventory Form (OGS
Form 1):
During your Homeroom Guidance Class this week, your class
advisers will hand you the Student Individual Inventory Form
(OGS Form 1) for you to fill in and accomplish. We request
everyone to complete the form fully and submit it to your class
adviser.

From the Office of Library and Learning Resource Center:


3. Accomplishment of Library Card (OLLRC Form 1):
During your LLRC Hour (or any designated schedule for
Senior High School), the school library and learning resource
in charge, Ma’am Alone Timenia, will hand you the Library
Card Form for you to fill in and accomplish. This card must
always be brought together with your ID card as you visit the
Library and Learning Resource Center.

From the Office of Student Discipline (OSD):


4. Implementation of the following OSD Forms, Slips, and Passes:
a. Attendance Form – Each class will be provided a RED
FOLDER intended for attendance checking for each class. It
will be the class adviser’s/learning facilitator’s duty to sign the
attendance form in their scheduled class. The Class Secretariat,
together with the Protocol Officers, will be keeping the red
folder.
b. Absent Slip – Should you be absent from attending the class,
you should first report to the Office of Student Discipline to
present a valid excuse letter (signed by your parent/guardian).
The excuse letter will be received by the OSD and a signed
Absent Slip will be provided to you in return. The said slip
should be presented to the Class Adviser/ Learning Facilitator
before admission to class after the absence. The slip should be
kept by the Class Secretariat for recording and safekeeping.
c. Tardy Slip – Should you be late to come to school class, you
should first report to the Office of Student Discipline, through
the School Security Force, to get your signed Tardy Slip. The
said slip should be presented to the Class Adviser/Learning
Facilitator before admission to class. The slip should be kept
by the Class Secretariat for recording and safekeeping.
d. Call Slip – Should someone related to you be looking for you
for an important matter, a call slip marked URGENT should
be presented by the School Security Force before you may be
allowed to go out of the class.
e. Corridor Pass – To prevent illegal loitering and ensure that
students are where they are supposed to be during school
hours, a Corridor Pass shall be issued by your respective class
adviser/learning facilitator. Typically, this pass is used for
various purposes, including restroom breaks, visits to the
school nurse, and other legitimate reasons for leaving the
classroom.
f. Gate Pass – For security measures that help monitor and
control the movement of individuals, particularly students,
within the school premises, a gate pass must be secured. It is
primarily used to prevent unauthorized exit or entry and
enhance the overall safety and security of the school. Gate
passes are available at the Office of Student Affairs or Office
of Student Discipline.

From the Office of Accounting and Finance Services (together with the
Office of Student Affairs):
5. Inclusion of Php 50.00 Student Organization Fee to your School
Fees:
With the increase of the ongoing student organization
activities led by the Office of Student Affairs, the SRHS
Administration Office, through the Office of Accounting and
Finance Services and Office of Student Affairs, will be
including the Student Organization Fee amounting Php 50.00
to your School Fees/Financial Accounts.
From the Office of Campus Ministry Services (with the Office of Student
Affairs):
6. Grade 11 – St. Gabriel the Archangel Recollection and Feast
Days:
a. The Grade 11 – St. Gabriel the Archangel class will have its
scheduled recollection and feast day on September 28-29,
2023. Thus, for the said dates, regular class for G11-SAG
students will be suspended to attend the needs of the
recollection and feast day.
b. The Class Adviser, Miss Novelle D. Alvarez, together with the
Class Officers, should closely coordinate with the Office of
Campus Ministry Services and Office of Student Affairs.

From the Office of Student Affairs – Student Organization Unit (OSA –


SOU):
7. National Science and Technology Month 2023
Theme: Siyensya, Teknolohiya, at Inobasyon: Kabalikat sa
Matatag, Maginhawa, at Panatag na Kinabukasan
Classroom-based Science Class Activities: These are activities
to be accomplished during the Science classes of learners:
 SciTechArt: A Digital Poster Making Contest
 InfoSciTech: An Infographic Making Contest
 Eyes on Science: Journey Through Discovery
 TechClick: Where Science Meets Shutter

School-wide Science Competition:


The best outputs to be chosen by the class together with their
Science learning facilitator and class advisers, will represent
the class in the school-wide competition.
 Science in Minute(s): The TechTok Experiment Contest
(for SHS STEM students)
Awarding will be done during the Flag Lowering Ceremony.

8. House of Music and Dance Auditions for Himig Rafaelians (The


SRHS Official Choral Group) and Kamanyang Dance Troup
(The SRHS Official Dance Group)
Auditions and practices will be held starting tomorrow, 4:30
PM onwards. Parental Consent Form is available at the Office
of Student Affairs.
From the Office of Student Affairs – Student Leadership Unit (OSA –
SLU):
9. Submission of ID Files per Section
The Class Secretariat is given until September 27, 2023
(Wednesday), 5:00PM to accomplish the files discussed in
yesterday’s Secretariat’s training.

From the Office of Administration (OAd):


10. 31st National Family Week
a. The classes will be suspended from 3:00 PM onwards today,
September 25, 2023, to celebrate the Kainang Pamilya
Mahalaga Day pursuant to Presidential Proclamation No. 326,
s. 2012.
b. Classes will be shortened following the 50 minute-class
interval:
i. First Period: 7:30 AM to 8:20 AM
ii. Second Period: 8:20 AM to 9:10 AM
iii. Health Break: 9:10 AM to 9:40 AM
iv. Third Period: 9:40 AM to 10:30 AM
v. Fourth Period: 10:30 AM to 11:20 AM
vi. Lunch Break: 11:20 AM to 12:20 PM
vii. Fifth Period: 12:20 PM to 1:20 PM
viii. Sixth Period: 1:10 PM to 2:00 PM
ix. Seventh Period: 2:00 PM to 2:50 PM
x. Classroom and Ground Improvement: 2:50 PM to 3:00 PM
xi. Dismissal: 3:00 PM
