Detail of accident Factors

The ducts on the roof had deteriorated and were about to be
replaced. The victim and worker A were carrying a replacement duct
(approx. 10 kg) on the roof. When they reached the duct to be
replaced, the victim was left to carry the rest of the duct, and worker A
took a break. When the victim was carrying the duct, he collapsed
through the roof and crashed to the factory floor.
1. The documents necessary for the implementation of the work were not
submitted to the construction planning department, and the work was
performed without a construction permit.
2. He was wearing a fall prevention device, but he did not hang a hook on to
the main rope and did not use foot boards.
Countermeasure
