The ducts on the roof had deteriorated and were about to be replaced.
The victim and worker A were carrying a replacement duct (approx. 10
kg) on the roof. When they reached the duct to be replaced, the victim
was left to carry the rest of the duct, and worker A took a break. When
the victim was carrying the duct, he collapsed through the roof and
crashed to the factory floor.
