Waiver for the Geological Field Study

I, the parent/guardian of __________________________________________ with SR Number _______________, hereby

give permission for him/her to participate in the Field Work as part of the Modern Biology course at Batangas State
University – The National Engineering University, located in Alangilan, Batangas City, Batangas, Philippines. The Field
Work is scheduled to take place in Laiya, Batangas on July 08, 2023, Saturday.

I am aware that there are inherent risks associated with travel and the activities involved in the Field Work. I understand
and accept that these risks may include, but are not limited to, injuries, allergies, diseases, or other physical harm to my
child, as well as damage to or theft of his/her personal belongings. However, I trust that the safety of the students during
their travel and throughout the activity is ensured by their licensed professional professor, who is also a certified Emergency
Medical Responder (EMR) and Safety Officer.

It is important to note that the Field Work is an integral part of the Modern Biology course. By participating in this hands-on
experience, my child will have the opportunity to apply theoretical knowledge gained in the classroom to real-life
situations. The Field Work will enhance their understanding of concepts, promote critical thinking, and foster practical skills
development. Moreover, it will provide them with valuable exposure to the natural environment, allowing them to observe
and analyze biological phenomena firsthand.

I acknowledge that the professor, as the lead organizer of this geologic field study, is not liable for any self-inflicted risks or
harm that may occur during the Field Work. I understand that my child will be under the supervision and guidance of the
professor and other authorized personnel throughout the entire duration of the activity.

By signing this waiver, I affirm that I have carefully read and understood the contents of this document. I am fully aware of
the potential risks involved in the Field Work, and I consent to my child's participation with the understanding that appropriate
safety measures will be taken.

For chronic health diseases or any medical problem, kindly attach a medical certificate acknowledging that your son or daughter is fit to
attend the Field Work, if available.
