RamalayamTemple, Georgepeta, Tallarevu Mandal, EastGodavari District, A.P, do hereby most
solemnly affirm and state on oath as follows

1.That I belong to OC- Kapu Community and I have faith and belief in God and profess
Hinduism and my religion.

2.that there is no criminal case pending against me in any of the Police station in the
State of Andhrapradesh.

3. That there is no Commissioner”s case are pending in any of the Court of Law pending
enquiry against in the State Of Andhrapradesh.

4.I am not convicted off any offences in the State Of Andhrapradesh.

5.That there is no Conviction or sentence in force.
