INTRODUCTION:

Natural disasters and other emergencies such as traumatic incidents, sudden death,
suicide, both attempted and completed, bombing, and shooting are situations that may occur at
any time. If a crisis or emergency may occur, an Emergency Management Plan was developed to
prepare and guide everyone in the school community to stay safe. This guide covers different
ways everyone can handle and prepare for any crisis or emergency in the school.

The SHS Emergency Management Team (SHS EMT) was formed as an interoffice team
to plan, manage, execute, and guide personnel and students in the event of a crisis or emergency.
The team is composed of: the School Principal; Assistant Principal for Administration and
Finance, Assistant Principal for Academics, and Assistant Principal for Formation and
Community Extension; members of the Senior High Emergency Response Team (S.H.E.R.T.);
University Security Office (USO); Office of the Prefect of Discipline (OPD); School Infirmary;
Guidance and Counseling Office (GCO); and, Physical Plant Office (PPO). This emergency
management plan enhances the Senior High School’s efficiency to respond immediately and
effectively.

RATIONALE:

On campus, a crisis or emergency is unavoidable. It is a situation that is out of the norm


and/or has the potential to cause significant disruption to the school’s normal operations. The
Senior High School Emergency Management Plan (SHS EMP) will help meet the needs of the
students and personnel during times of crisis or emergency inside or outside the campus. The
SHS EMP desires to ensure the physical and mental health, and safety of the community while
providing an environment that is conducive to healthy learning and safety for learners and
employees.

Major Goals:

1. To facilitate the safety of the students and personnel;


2. To avoid confusion;
3. To help the school community survive a traumatic situation;
4. To help the unit get back to normal operations as soon as possible;
5. To reduce psychological suffering;
6. To protect students, personnel, and visitors from unreasonable risk or harm; and
7. To guide actions of intervening first responders.

EMERGENCY INTERVENTION GUIDELINES

It is the objective of the emergency intervention guidelines to provide a basis for, and to assist in:

ü Administrative support to this effort by allocating staff time and resources to the
development of other safety guidelines on the campus;
ü Recommendations for daily routine security and safety on campus;
ü Planning for training for all personnel and volunteers involved in Senior High Emergency
Management Plan on their roles in an emergency; and
ü Adoption of a plan to address both internal situations and those requiring support from
external services (Central Police Station 166, ZC Police Station, ZC Fire Department,
Brent Hospital, Ciudad Medical Zamboanga, West Metro Medical Center, Zamboanga
Doctor’s Hospital, ZC Medical Center, and Zamboanga Peninsula Medical Center)

School Principal Incident The Incident Commander is responsible for all aspects of a

EMERGENCY RESPONSE CODES

Code Blue Medical Emergency


 Illness or disease (e.g., most common examples are
hyperventilation, asthma, flu, dengue fever, epilepsy, cardiovascular
conditions, etc.) with physical manifestations, such as chest pains,
fainting, vomiting, high fever, whooping cough, convulsions,
vertigo, etc.
 Accidental injuries (e.g., fall, fracture, lacerated wound, avulsion,
burn, etc.)

Code Yellow Internal Emergency


 Explosion, natural disaster (earthquake, typhoon), engineering
failures (burst water mains, loss of electrical power, gas leaks, air-
conditioning failures, elevator failures)
 Impacts on building or grounds; incidents in the immediate
surroundings of the university
 Information and communication failures

Code Orange Evacuation


 Movement of students, faculty, and staff within or from building
premises in as rapid and safe manner as possible; removal of people
from immediate danger area to a safe area

Code Red Fire/Smoke


 Fire alarm activation / Presence of fire

Code Purple Bomb Threat / Arson Threat / Bombing Situation


 Possible threat from a suspected material or an explicit via lines of

STAGES OF ACTIVATION

In an emergency, the School Principal (during office hours) or the USO (off hours) shall assess
the emergency, declare the code activation, and execute the code stages. Depending on the type
of emergency, he/she will determine if the response needs to be elevated to a higher level of
university emergency management. The School Principal assigns all three (3) Assistant
Principals to manage and supervise the entire process of emergency management. After a full
response from Stages 1 to 4, the Assistant Principals will coordinate a major debriefing session
with the Senior High Emergency Management Team and the relevant officers of the emergency
teams.

Stage 1. ALERT Notice of an emergency shall have been given. The School
Principal and USO have been notified of a situation that could
escalate or that may require coordination of support resources.

Stage 2. STANDBY There is an increased likelihood that the emergency will have a
major impact on the services and operations of the school.
Stage 3. CALL-OUT An emergency exists. Alarms sound of the OPD at the Guard area
in the lobby, the Administration Office, and the School Infirmary.
Upon receipt, the Senior High Emergency Response Team will
need to be dispatched and sent to the location of the emergency.
Stage 4. STAND-DOWN The emergency has been contained, and emergency operations are
no longer required. The school shall resume normal operations and
activities. Debriefing by Assistant Principal for Formation and
Community Extensions follows with the help from the Guidance
and Counseling Office (GCO).

CANCELLATION OF CODE
The School Principal (during office hours) or the USO (off hours) will determine and decide
when a code is to be canceled. Cancellation will be initiated, and an “All Clear” is announced
with the type of emergency code declared. For example, if a suicide attempt led to a decision to
evacuate, Code Grey A, then Code Blue and Code Orange would be given. If the emergency
has been contained, then the call would be “Code Grey A, All Clear” followed by “Code Blue,
All Clear,” and “Code Orange All Clear.”

In case the School Principal is not available, the next authorized school administrator to activate the codes will be
the three (3) Assistant Principals, and if anyone of them is not available, the designated USO and Safety Officer of
the Senior High School.
CODE GREY: Suicide Emergency Response Measures

Suicide prevention is the concern of not only mental health professionals but also the whole
community. Faculty and staff play a key role in the early detection and intervention of people
who are at risk of self-injury and suicide.

In the poll conducted by 6UNICEF, the emotional, psychological, and social crisis is having an
immense impact on the young generation and adults. Anxiety, depression, suicidal behavior, and
non-suicidal self-injury are slowly taking their toll. Given the increasing concern for mental
health in the community and the need to establish a mental health procedure according to the
new mental health law, a mental health protocol in the school needs to be created and
implemented.

Every school must respond to a suicide crisis carefully and thoughtfully to diminish the threat of
the immediate situation, speed recovery, and bring about a return to normalcy for the school and
community.

A. Suicide Ideation and/or Suicide Threat

Suicide ideation or thought refers to the preoccupation with the ideas about suicide. This
is where a person repeatedly thinks of suicide and what life would be like if he/she is not around.
A suicide threat is a verbal or non-verbal signal that a person plans to hurt themselves to
die but has not acted on the conduct.
** Please refer to the Warning Signals for Suicide from “Guidance – Dealing with Non-Suicidal
Self Injurious Behavior (NSSI) and Suicide in School: Prevention and Intervention Manual.” **

Guidelines to follow in case of a Suicide Ideation and/or Suicide Threat:


The following are guidelines for dealing with students who express a desire to harm
themselves. When the risk of suicide exists, the situation must be managed by the Guidance
Counselor and/or school nurse. Under no circumstances should an untrained person attempt to
assess the severity of the suicidal risk. All assessments of threats, attempts, or other risk factors
must be left to the appropriate professionals (i.e., guidance counselors, social workers,
psychologists, mental health therapists, and school nurses).

In cases of suicidal risk, the school should maintain a confidential record of actions taken.
This will help assure that appropriate assessment, monitoring, and support are provided as well
as document the school’s efforts to intervene and protect the student. A standardized screening
tool or form may be used for this purpose and then signed by the attending counselor. It is
recommended that the forms be readily available for reference/documentation.

Call the guidance counselor. If there is no available counselor, call the Teacher/Staff/Students

leave the student alone. Provide constant adult supervision while inside
the campus.
In case of injury or if with symptoms of hysteria or panic attack, notify Teacher/Staff/Students
III the OPD and inform the School Infirmary to provide First Aid or
immediate medical intervention.
Maintain a safe environment for the student concerned until the student is Safety Officer/USO
escorted to the Guidance and Counseling Office or the Infirmary. Take

immediate action to isolate the individual posing a threat and prevent
access to potential weapons (if known).
Assess the seriousness of the threat. In case of a life-threatening situation, GCO, OPD, and
the student and faculty/staff involved must understand that the issue of Infirmary
VI confidentiality shall no longer apply. Determine risk and intervention
needed by interviewing the student, and gathering appropriate supportive
documents from teachers and others who witnessed the threat.
Guidance and Counseling Office will: GCO
a. Utilize the standardized assessment tool and Suicide and
Depression Screening Tools to assess the student;
b. Supervise the student and provide proper counseling;
c. Contact the parents/guardian and advise them of the situation and

make recommendations;
d. Offer support to the student and the family, letting them know
specifically the services which the school can offer; and
e. Place all recommendations in writing and make sure that the
parents/guardian receives the recommendation by signing off.
If the student is known to be currently in counseling, inform their GCO

treatment provider of what occurred and the actions taken.
Should the student reveal issues of parental abuse or neglect, the School GCO
Principal should be notified along with a discussion on possible
contributory factors in the suicidal ideation.

The School Principal coordinates with the VP for Basic Education and the
University President. Afterward, the school will coordinate with DSWD
as soon as the investigation is over and a final decision has been made.

b. Submit a psychological/mental health certificate to the GCO and School Infirmary
indicating student is psychologically fit to attend classes.

B.1. CODE Grey A: Suicidal Act or Attempt on School Grounds or During a School-
Sponsored Activity
Suicidal act (also referred to as suicide attempt) – a potentially self-injurious behavior for
which there is evidence that the person probably intended to kill themself; a suicidal act may
result in death, injuries, or no injuries.

Steps to Follow in Case of Suicide Attempt:

1. ALERT and EMERGENCY CALL OUT:

SHS personnel on the scene must call for help from another staff member but NOT leave
the student/personnel alone.

a. Do not PANIC. Locate the individual.


b. Inform another staff/student to call OPD (local 3034).
c. Staff states the following to the OPD:
ü Location of Emergency (Building, Floor, Room Number/Office)
d. OPD notifies School Principal and together declares that it is a Code Grey A
(Code Grey-Alpha).
e. Security Guard on-duty call out “Alpha Base” (University Security Office, local
1015) and “Medic One” (Infirmary, local 3041) for the emergency call.
f. Medical Team is dispatched to the location.

2. RESPONSE: On-scene incident

such a plan. (Refer to pages 16-17)
Any student/employee who might have witnessed the attempt will be

B.1. CODE Grey A: Suicidal Act or Attempt which occurs OFF School Grounds
When a school becomes aware that a student has attempted suicide off school grounds, the school
must protect the person’s right to privacy. In this case, staff response should be focused on controlling the
spread of rumors and minimizing the fears of fellow students and staff. Therefore, follow this procedure
only if a significant suicide attempt occurs off premises when one or more of the following conditions
exist:
1. Rumors and myths are widespread and damaging.
2. Students witness police action or emergency services response.
3. A group of the attempt survivor’s friends are profoundly affected by the suicide attempt.
4. Is publicized in the media.

PARENTAL COMMUNICATION: Contact with parent/guardian should be made by phone or in


person by the school principal, a SHS EMT member, and/or Guidance Counselor. The School Principal
will offer support to the family, letting them know specifically the services to which the school can refer.
Talk with the family and student about how they would like the school to respond in terms of notifying
teachers, other staff and students. [Discuss risks of widespread rumors]
PARENTAL PERMISSION: To ensure that correct information is being shared, and to obtain
permission to share that information, the parent/guardian of the student should be consulted. In the
absence of parental/guardian permission, care must be taken in the handling of information, unless it has
become official public information. In the absence of confirmation that this is a suicide attempt, but
widespread knowledge of the matter requires that it be dealt with, the information should be handled as an
injury.
MEDIA: Media representatives will be referred to the ADZU Communications Officer. Any SHS
personnel should not make official statements to the media/social media except through the SHS
Communications Officer or University Communications Officer.
PROVIDE CARE TO OTHER STUDENTS:
a. Have Guidance Counselors talk with the most profoundly affected friends and determine the type
of support needed.
b. Provide a counseling room. The Counselor provides necessary passes or excuse slip to release
these students from class to receive counseling services.
c. If deemed necessary/appropriate, members of the SHS EMT may follow the student’s classes
throughout the day providing counseling and discussion to assist students and teachers. This could
also help further identify and refer students who may be “at risk.”
d. Provide support to school-age siblings if allowed by parent/guardian.

C.1. CODE Grey S: Death by Suicide of a Student/Personnel on School Grounds or During a


School-Sponsored Activity

Suicide is death caused by self-directed injurious behavior with any intent to die as a
result of the behavior.

Note: The medical examiner’s office must confirm that the death was a suicide before any
school official may state this as the cause of death. The school must adhere to the wishes of the
family in this regard and respect their privacy and confidentiality.

The following steps will be followed in case of suicide:

1. ALERT and EMERGENCY CALL OUT: School personnel on the scene must NOT
PANIC and must call for assistance from another staff member or student and not leave
the individual;

Security personnel will verify and obtain as much factual information as
possible via school security, parent/guardian, or others who may have
facts depending on circumstances
Immediately notify, regardless of the day or time, the School Principal, OPD
and the three (3) Assistant Principals.
Promptly collect and safeguard the student’s belongings (from classroom OPD
or lockers), any student work, or photos. Consult with family members
and determine a mutually agreeable date and time in private, to return
these belongings.
VII The School Principal notifies with the VP for Basic Education and the School Principal
University President of the current situation.
VIII Inform the faculty and staff that sudden death has occurred using written School Principal/AP for
communication, followed by a unit or departmental meeting after the day. Formation and

If the death occurred in the evening, convene a staff meeting prior to the Community Extensions
start of the next school day.
Designate space for all postvention activities. AP for Formation and
Community Extensions
Once all appropriate information are gathered, provide for funeral AP for Formation and
arrangements and related details to students and school personnel. Community Extensions
Meet with the SHS EMT at the end of the day or days during crisis AP for Academics
management activities to ensure the exchange of important information,
as well as to ensure communication and further planning of school
activities.

Thank those who assisted in the postvention. Do not forget to include the
faculty, auxiliary staff, Emergency Response Team, and any outside
agency or community.
XI Any student/employee who might have witnessed the attempt will be OPD and GCO
promptly referred to the GCO for debriefing. Counseling can happen for
further processing, if needed.
XII Document all information received and actions taken. GCO, OPD, USO, and
Infirmary

Officer. Any SHS personnel should not make official statements to the Officer/ADZU UCO
media/social media except through the Communications Officer.

C.2. CODE Grey S: Suicide of a Student/Personnel which occurred OFF School Grounds

When a school becomes aware that a student has died by suicide off school property, the school
must protect the family’s right to privacy. In this case, staff response should be focused upon controlling
the spread of rumors and minimizing the fears of fellow students and staff. Therefore, follow this
procedure only if a significant suicide attempt occurs off premises when one or more of the following
conditions exist:
1. Rumors and myths are widespread and damaging.
2. Students witness police action or emergency services response.
3. A group of the attempt survivor’s friends are profoundly affected by the suicide attempt.
4. Is publicized in the media.

The School Principal and SHS EMT will direct all steps in this procedure and will document every step of
the procedure in the handling of information about the suicide.
PARENTAL COMMUNICATION: The School Principal will offer support to the family, letting them
know specifically the services to which the school can refer. Talk with the family about how they would
like the school to respond in terms of notifying teachers, other staff and students. [Discuss risks of

D. Actions to Avoid*

1. Do not announce the death of anyone over social media.


2. Do not announce over the radio the word or phrase “suicide” or “suicide attempt.” Use either
Grey A or Grey S.
3. Avoid canceling classes or pre-planned activities unless necessary; students find comfort in

CODE BLUE: Medical Emergency


A medical emergency is caused by an illness or disease (e.g., most common examples are
cardiovascular conditions, diabetes, epilepsy, flu, dengue, fever, schizophrenia, etc.). Illnesses or
diseases would at the time have physical manifestations, such as fainting, vomiting, high fever,
whooping cough, convulsions, vertigo, etc.

Medical emergencies also refer to accidental injuries (e.g., falls, fractures, lacerated
wounds, avulsion, burns, etc.)

Note: It must be noted that other types of emergencies can eventually lead to the need for
medical attention. Code Blue is differentiated from other emergencies in that it is primarily
medical in nature.

Step 1: Medical Emergency Alert (Call “3041”)

A. The Infirmary is notified.


B. For any medical emergency, please call 3041 or 0906-077-4184 or call the Guard-on-
duty assigned at the SHS Lobby Area. Provide the following information:

Where - room/office /floor/building


What - fainting: reason unknown, LBM, vomiting, physical injury, etc.
Who - teacher, student, staff, unidentified (e.g., visitor); male or female
How many- one, four, seven
Condition - ambulatory/can walk. Conscious but cannot walk, unconscious, etc.

C. People near the emergency must be made to immediately vacate the area. For
example, if it happens in a classroom, the whole class except the teacher must be
made to leave the room. If it happens outside of a classroom, the area must be
cordoned-off to keep the crowd away from the patient. Crowd control is vital as
crowding impedes emergency response. The guard in the area is required to provide
crowd control.

D. The patient should not be moved until the duly designated Infirmary Nurse/medical
personnel arrive.

Step 2: Emergency Call-Out

A. The Guard-on-duty calls our “Alpha Base” (University Security Office or USO) and
OPD through the radio or goes to the OPD office, which is just across the SHS
Security Guard’s Lobby post/station, for the “Emergency Call-Out” and gives the
following information:

The School Principal or Office of the Prefect of the Discipline (office hours) or Chief
of University Security Office (off hours) declares a “Code Blue,” followed by the
floor level and the room number, e.g., “Code Blue, 3rd Floor, Room 303.”

B. Alpha Base calls 3041 (Infirmary) to send the Nurse and the Doctor (if available) to
the specific location. The radio call sign of the Infirmary Staff is “Medic One,”
“Medic Two,” and “Medic Three.”

C. Infirmary calls Auxiliary for standby car and driver, if available.

D. Infirmary calls hospitals, SAFE, or HEMS for ambulance assistance, if needed.

Code Blue B: 4-6 patients

Step 3: Emergency Response Team

A. The Senior High Emergency Response Team is notified.

B. The Infirmary Staff is informed and proceeds to the area with a first aid kit bag, cell
phone, and a radio.

C. The Security Guard in the location secures and cordons, if needed, the area. They also
assist the medical staff with the stretcher or the wheelchair.

D. The Nurse/Medical Doctor decides whether the patient will be brought to the
Infirmary or a hospital.

o The Nurse calls the hospitals, SAFE, or HEMS for ambulance assistance, if
needed.
o The Nurse calls the Auxiliary Office (local ___), if standby University vehicle is
available.

E. The patient is brought to the Ground Floor to the Infirmary, University vehicle, or
Ambulance for treatment.

F. If the patient is a student, the Office of the Prefect of Discipline (OPD) will also keep
a record of all the information about the incident. If the patient is an employee, the
HRADO is notified of the incident.

Step 4: Hospital

A. The Infirmary Nurse will assess the patient and will provide necessary first aid.

B. The Infirmary Nurse informs the patient’s parents or guardian of the situation of the
patient immediately.

C. The Infirmary calls the hospital (hospital of choice by parents/guardians, if requested)


in advance regarding the patient who will be brought to the Emergency Room (ER)
from the campus.

The coding (A to C) is needed to give the medical personnel an idea of the quantity and type of materials,
equipment, kits, etc., to bring.
o The Nurse calls the hospitals, SAFE, or HEMS for ambulance assistance, if
needed.
o The Nurse calls the Auxiliary Office (local ___), if standby University vehicle is
available.

D. The Infirmary Nurse will accompany the patient to the hospital and will inform the
ER personnel that the patient is a student of the Ateneo de Zamboanga University
Senior High School. If AdZU Personnel, the Infirmary Nurse will request for a
guarantee letter from the HRADO.

E. The Infirmary Nurse will properly conduct the medical endorsement to the ER
Personnel of the hospital.

F. For accident emergencies, Infirmary will provide incident reports for Insurance
claims and will advise the parents or guardians of the Student Insurance.

G. The Infirmary Nurse on duty can leave the patient’s bedside only when the patient’s
parents/guardian arrives.

H. Proper documentation will be recorded in the patient’s health record in the School
Infirmary.

Step 5: Emergency Stand-Down

A. The Infirmary Nurse on duty files a report of the medical emergency; S.H.E.R.T or
OPD calls for “Code Blue, All Clear.”
B. Follow-up on the patient’s condition either through the parents/guardians, or when the
student is back on campus.
