Remote Inspection on site

Dear Sir,

To ensure high level of efficiency and safety in our operations, especially in areas where there is restriction to
perform physical inspection, Bureau Veritas added remote inspections to its breadth of services. Within the
scope of this remote inspection, we are using a dedicated remote application involving Bureau Veritas
inspectors and clients directly in the field.

For this purpose, you should:

 Authorize by this form the performance of this remote inspection and ;

 Make available the resources and equipment as detailed in attached document that will be used by
your representative on site to connect to the remote application on a smartphone/tablet connected to
the internet under the instructions of our inspector.

During the inspection, your representative remains solely responsible of the inspection site and you should
ensure high standards of safety on site and notify as any other particular hazard to Bureau Veritas Inspection
center and remote inspector prior starting remote inspection.
Moreover, please note that photos and video streaming could be collected during the inspection exclusively
for the purpose of the verification of the export and for use by Bureau Veritas within its assignment and by the
local Authorities of the country of destination.

The conditions under which Bureau Veritas will perform the processing of this data are set forth in its personal
data protection policy available on the following link: https://personaldataprotection.bureauveritas.com.
By signing this consent form, you agrees that your consent shall remain in effect as long as the request for
certificate is pending, and is effective regardless of whether you are present at the time of the inspection.
Failure to grant consent for this remote inspection may result in, denial of the certificate sought.

By signing this form, I certify that I am authorized to give consent to Bureau Veritas as described above.
