I, MANDA RAMA VENKATA RANGA RAO, ( aadhaar No. 6570 3206 2466 ), S/o SUBHASH CHANDRA BOSE,
HINDU, AGED ABOUT 42 YEARS, RESIDENT OF H.NO 6-104/1, NEAR WATER TANK, PETA KALIDINDI, KALIDINDI
MANDAL, ELURU DISTRICT, A.P do here by solemnly affirmed state on oath as follows.
1. I am an Indian.
2. I fully acquainted with the facts of this affidavit.
3. That my wife Late komaravolu Siva Durga died intestate on 15/09/2011 ( one, five, zero,
nine, two, zero, one, one) She died due to road accident. While she alive she worked as a
S.G.T at Pedalanka, (D.W).
4. Through my wife I have only daughter her name is MANDA SAI VENKATA ARUNA SRI.
Being minor legal guardian by father MANDA RAMA VENKATA RANGA RAO. Now my
daughter is studying 9th class student.
5. Now my daughter will claiming Family Pension benefit, I have no objection if District
Audit offices relies death benefits of my deceased wife.
