Bachelor of Surgery (MBBS) degree at Sultan Abdul Halim Mu'adzam Shah International Islamic
University , I am writing to request permission to receive a vaccination that is a mandatory
requirement for my program.

scheduled to receive the required vaccination on 29 October 2023 at Hospital Sultanah Bahiyah. This
