MEMORANDUM ORDER FOR MANDATORY TRAINING (MO-1777)

Purpose: For compliance of knowledge based training for all telecommunications company
regarding the Data Privacy Act of 2012 and RA11934 known as the SIM card registration law of
2022

Duration: Training Seminar (5 Working days)

Implementation Coverage : All Telecommunications Company governed by the Republic of the

Philippines

Implemented to: All Telecommunications employees from corporate to field offices

By signing this Acknowledgement, I confirm that I agree to the training class listed
above in its entirety. I listened, read, and understood that during the specified Certification
Training NTC and DICT will be verifying my criminal records Trainee SecurityDeposit in the
amount of P 5360.00. I also agreed to submit my NBI Clearance, Police Clearance and local
Barangay Clearance to my designated company for verification. The said amount will be in the
form of Cash Bond while we are waiting for the submission of the Criminal Liability Clearances. I
understand that as a newly hired employee, it is my responsibility to abide by the National
Telecommunications Commission and Department of Information and Communications
Technology’s policies and procedures, in accordance with the training.

This is for strict compliance of all Telecommunications employees and employers. And
must be implemented upon receipt of this memorandum.
