Minimum Qualifications for Admission

1. Applicants seeking admission as a freshman to the UERM Doctor of Medicine


degree program must have the following qualifications:
1. Holder of a baccalaureate degree
Applicants who have not yet graduated must be able to submit a
certificate of candidacy for graduation issued by his/her higher
educational institution.
He/She must have graduated on or before the regular enrollment
period of the UERM College of Medicine.
2. Must have taken the National Medical Admission Test (NMAT) not more than
two (2) years from the time of admission with a percentile score of 70 or better
To be admitted as a freshman for School Year 2023-2024, the valid NMAT scores are
those taken on the following dates:

The application of the following applicants will be processed only upon
signing, and complying with, the Undertaking for NMAT: (NMAT) Form
1. The current NMAT percentile score does not meet the cut-off of
the UERM College of Medicine
2. Yet to take the NMAT

1. Local Applicants
The following are considered as local applicants:
1. Resident Filipino citizen
2. Filipino with dual citizenship [with recognition and proof of
residency of at least five (5) consecutive years in the Philippines]
3. Application Requirements
1. For Both Local and Foreign Applicants
1. One (1) original copy of the birth certificate
2. One (1) original copy of the official transcript of records issued
by the applicant’s higher educational institution
3. One (1) original copy of the NMAT percentile score
4. One (1) original copy of the certificate of good moral character
issued by the applicant’s higher educational institution
5. Recommendation letter from two (2) of the applicant’s
professors in college
6. One (1) certified true copy of the diploma or one (1) original
certificate of candidacy for graduation issued by the applicant’s
higher educational institution, whichever is applicable
7. Two (2) printed copies of the signed online application form
8. Two (2) pieces of 2 x 2 ID picture with name tag (in white
background)
9. One (1) printed copy of the accomplished Undertaking for NMAT,
if applicable (NMAT) Form
10. Vaccination Card (PHOTOCOPY)

Online Application Procedures


1. Ensure that your e-mail address (preferably GMail) and mobile number
are active and working.
2. Accomplish the application form online through this link: Application

Review all the information you supplied before finalizing the form. Once
finalized, you can no longer edit the form. Ensure that you affix your
signature to the printed copies of the completed online application
form
3. Submit through courier the printed copy of the signed online
application form together with the documents contained in a long plain
folder and long brown envelope.
A dropbox will also be made available in the emergency exit gate of the
North Campus.
4. Upon receipt of your documents, the Office of Admissions will send an
acknowledgment receipt, through e-mail, of the checklist of documents
we received from you.
5. Instructions on how to pay the non-refundable application fee will also
be sent through e-mail. The non-refundable application fees for
admission as a freshman to the UERM College of Medicine are as
follows:
1. Local applicants – ₱3,000
2. Foreign applicants – US$300
6. Upload your proof of payment through this link: UERM UPLOAD PROOF
OF PAYMENT form
7. After validation of your payment, you will receive an e-mail and text
message on how to select the date and time schedule of your online
interview.
8. The successful applicants will be informed in the UERM Applicants’
Portal. They will also be notified through text message (for local
applicants) or e-mail (for foreign applicants).
