RESEARCH-INFORMED ASSENT FORM

The researchers are formally writing this letter to conduct a survey regarding the Influence of Nursery Rhyme
Videos on YouTube on a Child's Phonemic Awareness. This study aims to determine the influence of nursery
rhyme videos on YouTube in developing a child’s phonemic skills in terms of: a.) identification of phonemes;
b.) blending of phonemes; c.) segmentation of phonemes; d.) addition of phonemes; e.) deletion of phonemes; f.)
manipulation of phonemes.

The researchers are inviting you to be part of this data gathering process because we believe that you have met
our qualifications to participate in this study. Specifically, we are looking for a child who is watching nursery
rhyme videos on YouTube, aged 8 years old and below, as the subject of this study.

On the other hand, a parent or legal guardian who has the important responsibility of providing information
about their child will be asked some questions concerning the child's early development, particularly their
phonemic awareness skills. The entire discussion will be audio-taped to help us accurately capture all the
insights. The collected information during this study will be kept confidential. Nobody will know or hear the
tapes except for the researchers of this study, and they will be used for academic purposes only. If there are any
problems encountered, the researchers will take responsibility.

You may opt to or not participate in this study. You are allowed to discontinue the survey in the event of
