Re: Mr James Hutton, DOB:25/03/1933

I am writing to request overnight post-operative care for Mr James Hutton, a 89-year-old


retired widow who was admitted on today and underwent corneal transplant surgery in
right eye under full block local anaesthetic.

Mr Hutton has no vision after operation for several hours as his right eye unable to open as
well as no sight in left eye. He needs clear eye shield to cover right eye at night to protect
from knocks while sleeping and dark glasses should be provided when outdoors if possible.
It would be greatly appreciated if you help Mr Hutton with mobility, showering and dressing.

In terms of his medication, Mr Hutton has been provided Chloromycetin Eye drops 1 drop
and Prednefrin Forte Eye Drop 1 drop for his right eye. Please note, Prednefrin Forte should
be shaken well before use and be given 1-2 minutes after Chloromycetin. In addition,
Tenopt Eye Drops 1 drop has been prescribed in his left eye only BD as well as Panadol 1g if
necessary (Max 4g/day). He has a history of HTN, for which he takes Atacand 4 mg mane
and Lipe 10 mg nocte.

Mr Hutton is being discharged at 10am on 11 June 2022, his son will transport and assist him
to appoint with eye surgery at 11am.
