Re: Ms. Shauna Mendes DOB: 13/01/1997

Dear Dr Erduardo,

Thank you for seeing my patient Ms. Mendes, a 25-year-old single lady, whose clinical
features are suggestive of bacterial keratitis of the right eye. Your expert management
of the patient would be highly appreciated.

Ms. Mendes visited me yesterday on 19/03/22 with the history of wearing old contact
lens three days back after which she developed profuse watering and foreign body
sensation in her right eye. In addition, she has been having redness and pain along with
difficulty in opening the right eyelid. Penlight examination revealed corneal ulceration
and scarring as well as rashes in her cornea. Today, tear sample for bacterial
examination revealed Pseudomonas growth. Hence, diagnosis of bacterial keratitis was
made and she was put on tobramycin 14mg/ml eyedrop one hourly, Cefazolin 50mg/ml
eye drop one hourly and prednisolone acetate 1% eye drop six hourly.

Ms Mendes is advised to not touch or rub her eyes and also avoid splashing water
directly on her right eye. She is also counselled to not wear contact lenses until
complete recovery.

Please note, Ms Mendes has a past history of seborrheic blephritis and is allergic to
dexpanthenol eye drops.

She also has a history of gastritis one year back for which she is taking pantroprazole
regularly. It should be noted that she is near-sighted for -2.5 in her right eye and -4 in
her left eye.

In view of the above, I am referring Ms Mendes to you for further management. Should
there be any queries, please donot hesitate to contact me.

Yours sincerely,
Doctor
