DECLARATIONS

(i) Dual Signature Declaration (if applicable): I state and declare that my signature as per
___________________(document) is my old signature while my signature as per ___________________
Customer Signature
(document) is my present signature which is affixed in Biometric Savings Account Opening Form cum
MITC. Please consider this as my present signature.

(ii) Dual Name Declaration (if applicable): I state and declare that my name as per PAN is
_______________________ and name as per _________________ (document) is _____________________.
I hereby agree and understand that my name for Biometric Savings Account will be as per AADHAAR. Customer Signature
I request you, therefore, to open my Biometric savings account with your bank on my aforesaid
representation as per the form duly filled by me and accept the documents furnished by me in support
thereof.

(iii) Vernacular declaration (if applicable): I confirm that the bank executive has explained to me all Customer Signature
the product features, charges and important terms and conditions in my preferred language and the
same has been understood and agreed by me.
(Applicable for customer who signs in language other than English)

1. The bank official has explained to me the terms and conditions applicable, detailed features and charges as per bank’s General
Schedule of Features and Charges (GSFC) and average quarterly/monthly balance requirement as applicable to the product
subscribed by me and I have agreed to the same.
2. 2. The bank official has explained to me the most important document as set out in the Bank’s account opening form including
the clause on consent to call and I have signed the same after understanding the terms contained therein.
3. I understand that I will be bound by and have to abide by the Bank’s general terms and conditions available on the bank’s
website www.kotak.com and savings bank account rules.
4. I understand that the Bank reserves its right to revise its service charges as laid down in the General Features Schedule and
Charges (GFSC) which is available on http://www.kotak.com
5. I have given consent to activate the ActivMoney feature on my account and, I have read and agree to all the terms and
conditions of ActivMoney.
6. I/We understand and agree that the Bank may at its absolute discretion, discontinue or suspend any of the services completely

or partially with or without any notice to me/us
7. I understand and agree that the Savings Account should be used to route transactions which are personal in nature. In the event
of occurrence of transactions that may be construed as commercial/suspicious in nature, the Bank reserves the right to
decline/reverse such transactions and freeze/close the Savings Account.
8. I authorize KMBL to use my Aadhaar details and core biometric information to authenticate and verify my identity. I hereby give
my free consent to link my Aadhaar Number to all my existing bank accounts and customer profile for the purpose of availing
the Banking Services for operations of account Proof of Identity : *E-UID (AADHAAR) (Account Name & Date Of Birth will be
as per AADHAAR)
9. Applicable for Existing CRN customer: I agree that my existing details available in the bank system will be updated as per the
details given by me. Also, I understand that being an existing CRN customer, my account no. will be shared with me along with
the personalized Welcome kit at my registered communication address post account opening
