1. I declare and confirm that I am not enjoying any savings bank facility with any other
bank/financial institution.
2. I agree to surrender the extra facilities available against my existing SB account and
shall abide by the terms and conditions of the Bank pertaining to the PMJDY (BSBD)
account.
3. I hereby state that I have no objection in authenticating myself with Aadhaar based
authentication system and consent to providing my Aadhaar number, Biometric
and/or One Time Pin (OTP) data (and/or any similar authentication mechanism) for
Aadhaar based authentication for the purposes of availing of the Banking Services.
4. I hereby authorise to use my linked Aadhaar enabled bank account for receiving
government payments across schemes that I am eligible using the Aadhaar based
authentication.
5. I hereby declare that all information furnished by me is true, correct and complete.
