Change of Schedule for Onsite Admission Process

Please be informed that your scheduled onsite admission appointment is moved from June 3, 2023 to June 5, 2023.
You may come to OCC from 9:00 AM to 4:00 PM.

Please proceed to the OCC Admissions Office and bring the Original copy and Photocopy of the following
documentary requirements for final evaluation.

1. Printed Copy of Confirmation e-mail on Online Application for Admission

2. Printed Accomplished Application Form (See attached file)
3. High School Card (Form 138)
4. PSA Birth Certificate
5. Certificate of Good Moral Character
6. Four (4) pieces 2 x 2 recent colored picture with white background and applicant’s name tag
7. Photocopy of High School Diploma (if available)
8. Proof of Residency from the Barangay
9. Voter’s ID of Parent/Guardian
10. 2 pcs Long brown envelope

Put all your requirements in a Long Brown Envelope

OCC implements strict health and safety protocols upon entry on the school campus. "Please bring your
own pen."
