PARENT’S CLEARANCE AND WAIVER FORM

This is to certify that I am allowing ___________________________ to attend the

CBA Acquaintance Party this coming September 30, 2023 at 04:00 PM. It is
understood that he/she will follow the safety protocols needed to avoid any
unwanted incident.

I fully agree to waive any responsibility on the part of Asian College Dumaguete
in case of any untoward incident that may happen to the Student during the
duration of the acquaintance party.

1. The signature/s in this document must match the signature of the

parents on file.
2. In an event that the parent cannot sign the waiver form, it must be signed by
the legitimate guardian, as evidenced by the letter of guardianship on file.
3. This document must be dated, signed, and submitted on Saturday,
September 30, 2023. One copy shall be given to the DPD Department, while
the other copy must be kept by the parent/guardian.
