BORROWER’S AUTHORITY TO DEDUCT

In connection with my loan application with the Lender, I hereby authorize/stipulate the following:

I hereby assign in favor of the Lender the sum of at least my Amortization every payday from my salary and other
remuneration from the Company.

1. In this regard, I authorize the COMPANY as my agent to deduct said amount from my salary and other remuneration,
starting on the First Amortization Date and every payday thereafter, more or less, or until total installment and/or
corresponding obligation of amounting to the Total Receivables is fully paid.

2. I also authorize the COMPANY to remit the same directly to the Lender without need of demand.

3. Upon notification by the Lender, the COMPANY may deduct more than the amount herein specified in order to update
my account(s).

4. It is expressly stipulated that where I apply for vacations/sick/maternity leave privileges, this authorization extends to the
deduction of the amounts due and payable by me within the period of vacation/sick/maternity leave. It is further stipulated
that where I apply for commutation of vacation/sick/maternity benefits the deduction for the amounts due from me and
payable to the Lender during the period of leave shall likewise be made.

5. It is likewise expressly stipulated that in the event of payment to me of terminal benefits, including but not limited to
retirement pay, pensions, separation pay, gratuities, upon resignation, retrenchment, separation or termination of my
services, and/or death I hereby further authorize the COMPANY to deduct from such funds, monies, properties accruing
to me such amounts that would fully extinguish my obligations to the Lender and that no amount of funds accruing to me
shall be delivered or remitted to me without fully extinguishing my obligation(s) with the Lender plus accrued interest
thereon.
