RELEASE & QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, MARIA THELMA ESPINOL CHAVOSO, single, resident of 7141C M. Ocampo St.,

Brgy. Pio Del Pilar, Makati City, do hereby RELEASE, REMISE, AND FOREVER DISCHARGE and
by these presents HAVE RELEASED, REMISED, AND FOREVER DISCHARGED said UNIVERSAL
LMS FINANCE & LEASING CORPORATION, its stockholders, officers, directors, managers,
representatives, successors and assigns, each and all of them, of and from any and all liabilities and
obligations of any kind whatsoever, benefits of compensation of whatever kind or nature, particularly
those arising from and under the new Labor Code and its implementing rules, Social Security/Phil Health,
and other laws, as well as presidential decrees and general orders issued by the President of the
Philippines which effect and/or have relation to or connection with labor and employment which I, my
heirs, representatives, successors and assigns ever had, now have, or may hereafter have against the
Company for, upon, by reason of, or a consequence of my having worked for said Company at one time
or another.

This release and quitclaim is given in consideration of the sum of PESOS: One Hundred
Three Thousand Eight Hundred Forty Six & 58/100 Only (Php 103,846.58), which will be paid to
me by UNIVERSAL LMS FINANCE & LEASING CORPORATION, receipt of which amount is
hereby acknowledged to my full and entire satisfaction.

I hereby further undertake not to disclose to any person or entity in any manner whatsoever any
information of any kind relating to the business of UNIVERSAL LMS FINANCE & LEASING
CORPORATION. I also undertake to surrender to the Company all facilities, equipment, paper and
documents which have come into my possession as a result of the engagement of my services by the
Company.

I hereby expressly declare and manifest that this RELEASE AND QUITCLAIM may be pleaded
as a bar to any suit or action before any court or government office, judicial or administrative, in the
Philippines.

IN WITNESS WHEREOF, I have hereunto set my hand on this 31st day of August 2022, at
Makati City, Philippines.
