AFFIDAVIT OF RELEASE, WAIVER AND QUITCLAIM

I, JUDY FRANCISCO PASTORPEDE, Filipino, of legal age, and a resident of Blk 26 Lot 271
Bulacan Meadows Brgy Vaypombo Sta. Maria, Bulacan, Philippines, after being sworn to in
accordance with law, depose and state:

1. That I am one the Complainant-Appellees in the cases of JUDY PASTORPEDE, et al.,

Complainant-Appellees versus POLYSTAR GENERAL SERVICES (“PGSI” for brevity), and
THE PACIFIC PLAZA TOWERS CONDOMINIUM CORPORATION, Respondents-Appellants
docketed as NLRC LAC No. 06-002102-19, NLRC NCR Case No. 07-12578-18 and NLRC
NCR Case No. 08-13718-18 (consolidated cases)

2. That I hereby acknowledge to have received from PGSI, the sum of

_____________________ which is in full and final satisfaction of my salary and
other benefits that may be due me for the service which I have rendered for PGSI;

3. That I hereby declare that I have no further claims whatsoever against PGSI, its
President, members of the Board, officers or any of its staff and that I hereby release
and forever discharge all of them from any and all claims, demands, cause of action of
whatever nature arising out of my employment with PGSI;

4. That I further agree that this WAIVER, RELEASE AND QUITCLAIM may be not pleaded in
bar to any suit or proceeding (Civil, SSS, PhilHealth, Medicare, Labor, etc.) to which
either I, or my heirs and assigns, may have against PGSI in connection with my
employment with the latter and that the payment which I have received as provided
herein should not in any way be construed as an admission of liability on the part of my
employer and is voluntarily accepted by me and will, if need be, serve as full and final
settlement of any amount(s) due to me or any claims or cause of action, either past,
present, future, which I may have in connection with my employment with PGSI;

5. That I hereby RELEASE and DISCHARGE with prejudice PGSI and its officers, owners
or stockholders from any and all claims, including but not limited to non-payment of
salary, service incentive leave, allowances, separation pay, moral and exemplary
damages and attorney’s fees subject matter of the abovementioned cases

6. That I final manifest that I have no further claim(s) or cause(s) of action against PGSI
nor against any person(s) connected with the administration and operation of the latter
and forever release the latter from any and all liability.
 7. That the contents of this Release, Waiver and Quitclaim have been explained to me in a
language known to me and I have voluntarily executed and signed this affidavit without
any force, pressure or duress being exerted on my person and with knowledge of its
effects and implication and with the assistance of the Department of Labor and
Employment thru the National Conciliation and Mediation Board.

8. That I respectfully move that the cases against POLYSTAR GENERAL SERVICES INC., be
deemed terminated and dismissed.
