RE: Request for Certification of Payment

The Caltrade Industries Co. Ltd., through the undersigned counsel,

JUSTINE JOY H. BULANON, respectfully requests your Honorable
Officer, DENNIS P. FLORENZA, to issue a Certification of Payment
for CALTRADE INDUSTRIES CO. LTD.’s penalty payment made on
October 27, 2023.

On October 27, 2023, Caltrade Industries Co. Ltd. transferred its

payment (BIR Form No. 0605) for the penalty through the Bank of the
Philippine Islands amounting to One Thousand Six Hundred Twenty-
Eight Pesos and Sixty-Two Cents (1,628.62) and One Thousand Pesos
Only (1,000.00) under the name CALTRADE INDUSTRIES CO.
LTD.

However, the said payment was not immediately reflected in the

company’s Taxpayer Identification Number due to a late issuance. On
November 10, 2023, following an inquiry with a BIR representative, it
was confirmed that the payment had been uploaded to the company’s Tax
Identification Number 637-305-004-00000.

Upon inquiry from a BIR representative, we were advised to file a

request letter and submit the necessary requirements in order for us to
receive certification of Payment as a proof of successful payment.
