    COTININE GUIDELINES
    • NEW BUSINESS - Applicant is applying for Non-Smoker rate. Refer to age and amount on
    the table of Other Medical Requirements.
    • POLICY CHANGE - Change rate from smoker to non-smoker rate. Get Cotinine tests on all
    requests regardless of amount and age.
    • CONVERSION OF TERM BENEFIT/PLAN TO A PERMANENT PLAN, NON-SMOKER RATE.
    • TERM BENEFIT ATTACHED TO OLD POLICIES ISSUED AT AGGREGATE RATES.
    Follow age/amount on Table on Other Medical Requirements.
    • TERM BENEFIT ATTACHED TO OUR SMOKER/NON-SMOKER POLICIES.
    Get Cotinine tests if rate will change from smoker to non-smoker rate.
