Introduction
- You will discuss your OWN IDEA of what is on-the-job training, importance of
OJT in your field of specialization, why you need to comply with the
course/subject, what are its significance for the enrichment of your knowledge
and skills in your chosen course. Properly acknowledge references/sources if
you have used literature/studies/articles.

Description of Cooperating Agency/Company

- This will include the brief history of the company, its site and location, its vision,
mission, goals, corporate value, rules and regulations, facilities and others.

Narration of Experience
- This should discuss your experiences from the first day up to the last day of
your OJT but the presentation of significant experiences must be in a weekly
basis (by paragraph), including your working relationship with your supervisor/s
or co-workers.

Problems Encountered
- This should discuss your problems encountered during the conduct of your OJT
or based on your experiences.

Conclusion
- This should discuss about your overall impressions in your work assignment,
your colleagues and the importance of OJT.

Recommendations
- This should discuss your recommendations on the problems encountered and
on the OJT program as a whole.

Professional Readings
- It should be related with the importance of OJT, at least five (5) with reaction
and complete reference citation.
Bibliography
- All references should be alphabetically arranged by author’s surname or title
of the books/magazine, observe proper APA format.
