AFFIDAVIT OF PARENTAL CONSENT

FOR MINOR CHILD TO TRAVEL

I, DIONEL T. GERONA, of legal age, Filipino Citizen and a resident of 54 A DAVAO

ST., PAEL ESTATE CULIAT QUEZON CITY, after having been duly sworn to in
accordance with law, hereby depose and say:

1. That I and my Live-in-Partner, PRINCESS MERLY T. AGUSTIN, is the biological

and legal parent of DARAH VENICE A. GERONA, was born on November 07,
2021 at DR. MONTANO G. RAMOS GENERAL HOSPITAL, QUEZON CITY;

2. I cannot personally accompany our child during her travel at ITALY and FRANCE on
July 17, 2023 to August 1, 2023;

3. That I am giving my full consent to my Live-in-Partner, PRINCESS MERLY T.

AGUSTIN, to accompany our daughter during her travel to ITALY and FRANCE;

4. That I personally guarantee the financial support of the child in the course of her
travel and stay outside the Philippines and further guarantee that she will not in any
manner become public ward of any authority of any country she may happen to pass
en route to her destination;

5. That this affidavit was execute for the purpose of attesting to the truth of the
foregoing facts and for whatever legal purpose it may say serve; and

Affiant further sayeth naught.
